depends on the terrain and what I feel like building... Sometimes I build elaborate and put my own touch on things and sometimes it is rustic just for a bit to get a general shape and function till I can think of something to make it more stylish I guess would be the word for it. I tend to plan things out, always having in mind the basic idea, while keeping that idea open so I can modify it if needed.
These days though in survival I always usually end up digging a hole in the side of a mound or cliff and build downward. I decorate slowly adding this and that when the "base" has the layout I want. Things like caves and water holes/lava chambers usually change my plans in a way that I just think of how I want to use that space. Finding a village I would spruce it up when I can, little by little till I have it the way I imagine it to look like.
I do tend to either modify the village housing or add some of my own to a village to add more color to it. In a way I build with the terrain and sometimes utilize it for inspiration. So playing on a flat world would not do for me. I don't mind a little flat areas as I can get inspiration from my own builds to add onto or make it all my own...:D

My First World, always getting back to is a pleasure I enjoy with each new update that brings in more things to add in.
i usually start with a smallish house, typically a main floor for the chests/furnaces/crafting table/etc. and an overhanging mezannine type thing for the bed and 1 chest for my iron/diamond/gold/harder to find stuff.
then as the game progresses and i need more space, i'll expand it so the end result feels like a bunch of structures tacked onto the initial smallish house...
i usually play on a 'public' server so a secret room would be my first expansion, and it'll typically contain my enchanting table and brewing set up... all underground behind a flush piston door of some sort and a lava trap.
then i'll start expanding upwards.
that's the typical development process that takes place over the course of a month or two.

As stated before, I go for all kinds of builds. It also depends on the biome and terrain. (At times will terraform to meet my demands) Yet when I do builds I try to make the building (ex: lodge) beautiful and detailed on the outside (say by adding potted window plants hanging or by having the path be trim with a nice border). Then I would be as aesthetic as possible inside. (With counters, tables, sitting areas, beam support)
In the ground. My current world I lived in a village house. Have a storage cellar near there with 15 chests in basically a big room. Found a Ravine a ways off, so built a base in a tunnel I found while digging out of ravine. Started with just a room, then cleaned up tunnel and dug down to lvl 12 and made a large slime farm.
I normally start with a 8x8x3 room dug into the side of a hill/mountain, spend a few days getting resources, then start building a home with the full intention of moving into it (even going so far as to add in secret rooms/storage areas) and after i get it finished/furnished, it usually ends up being "abandoned" and i stay living in my 8x8x3 hole....
Nice buildings that have a lot of windows and looks reaslistic and has enough room to look good. I try to mesh my houses with survival supplies whiole trying to keep it good looking and very visible from the distance, although I already know mostly where all my stuff is about 2 hours into a new world, so it's quite useless. Oh well.
I start out with a smallish house on the surface, then when it feels crowded I dig down and make a large basement that I use for smelting, enchanting, brewing, item storage, and my bedroom. I don't like it crowded, but I also don't like it too spread out.
I've been trying to be more creative recently and building larger houses with more of a design element to them, but I still end up with a lot of small to medium size houses that are simply functional. Big enough to hold beds, chests, furnaces, and they keep monsters out.
I'm a hermit. I tend to just dig out a cave, and build my home into the hill/mountain. I'm currently trying to build my mountain into a castle, with mild success.
