Big issue. I exited my game earlier, and not in Creative Mode either never put this particular seed into Creative Mode at all. And it seems my game is now loading in Creative mode, but I tell it to load in Survival Mode.
I have no clue what's going on here. Has this happened to anybody else?
The load screen will generally display that last mode you've used in ANY world. So, if you entered another world in Creative, it may be displaying Creative as the last mode you've used. Toggle the mode button back to where it says Survival (by pressing A while that button is highlighted); and it should then load the world in Survival.
In the main menu when you select your world, there's the options screen where you'll see the game mode option, it's either CREATIVE or SURVIVAL, probably it's set as CREATIVE for now so just change it to survival, load the world and play it normally.
Did you try to enable host privledges such as invisibiltiy and flight? If so you will received the Creative mode message even though you did not actually use creative mode. 4J considers the use of host privledges to equate to creative mode and once the world is saved with these on, it will remain with the creative mode warning forever.
I guess you guys overlooked this part of the post.
No this hasn't happened to me Rose, I hope you get it figured out. You can try clearing your system cache, then re-updating. See if that helps.
I did see it, but I thought that double checking the toggling first wouldn't hurt... before perhaps going to more extreme measures (like clearing the caches and redownloading updates). Another possible issue may be the controller itself. If the contoller is a little old or dirty (i.e. sticking a little), it could be inadvertently toggling both back and forth with a single press of the A key.
Electricblooz (above) has also tapped into another possibility. The message "created in survival mode" changes to "created in creative mdoe" if host privileges have ever been activated in a survival world. While I personally feel this makes the message unclear, the "logic" behind it is that activating host privileges disables the leaderboards and achievements in exactly the same way as loading the world in creative mode does. Perhaps the OP is misunderstanding this particular message.

Yes, you should see the world "creative" or "survival" on the toggle itself before you select load. If, however, the controller is sticking, this can flip fairly quickly and may not be noticed before the load button is pressed. (I do have some personal experience with a sticky controller and having selections flip on me just before activating the load screen.) In addition, some people I know (my friend's kids, for example) can also tend to flip through load menus very quickly without REALLY reading them... and wind up activating options they never intended to activate.
I'm not saying that this is necessarily what has been happening to the OP. It may well be just as you suggested. However, you ARE suggesting a more drastic solution than I; and, in computing, it's usually better to fully investigate the simple stuff first... then proceed, if necessary, onto the more complex scenarios. Some people who have recently deleted updates have had issues with redownloading them.
You initially commented that must have missed the OP's statement and I was merely indicating that, at least from my perspective, that particular assumption was not an accurate one.

Could also be a heat fault in the controller - perhaps also caused by dust/dirt/hair inside it. Perhaps get a air can and blow it out a bit.

Do you happen to have an older Xbox model? As they age they tend to generate siginificantly more heat and this causes issues.. Not to mention other things. I'm too cheap to toss my arcade so I have quite a few lag issues just because my Xbox's processor can't handle things sometimes.
