I'm sure all the Fallout: New Vegas veterans out there remember the radscorpions.
(Tangent, but bear with me here)
Radscorpions, in the Fallout mythos, were pretty much scorpions that had grown to a colossal size and were extremely aggressive. They were usually a minor difficulty in the early Fallout games.
In Fallout: New Vegas, they got a major upgrade. They became extremely tough and extremely deadly. Worse, they made your life very difficult at low levels, as they pretty much were unstoppable juggernauts.
In fact, you usually spent those low levels in constant fear of one of those monstrosities crawling over a nearby dune. Even seeing them ruined your day.
But I realized this was part of the game's challenge. I think it IS a boon to a game to have a virtually unstoppable enemy you are praying not to meet. What's more, it would add more sanctity and security to want to build a house, where you'd be safe.
Pretty much all the monsters in this game are very easy to deal with. What if we had an overland dragon (only one) who was bad news, that you couldn't take on until you were ultra powerful? Perhaps have him spend nights in a lair, but spend the days wrecking havoc on the world?
