I'm not in a spot where I can provide all the info regarding my seed, but hopefully I can do that later today. I'm having this issue as well. I've also found that this seems to happen more often closer to the edges of the map. I was digging the map out near the SW corner of the map and had a ton of issues. It is better the further I get away from the edges of the map. The issues seem to be confined to chunks. Although I'm assuming that two adjacent chunks could experience the same issue. And thinking about this now, all the chunks that seem to be having an issue are in a snow biome. The SW corner of my map is mostly snow.
I did find that if you place a torch on the block that reappeared, it will cause the block to disappear.
I've tried playing both offline and online. I've not had anyone join this world at any point, nor have I played split-screen on it.
Just started getting the reappearing pickaxe bug in the 1.0.1 tutorial world. Used an enchanted Unbreaking I pickaxe on my cobblestone generating until it broke. Switched to a second pickaxe, but the first pickaxe reappeared and I was able to mine 3 more blocks with it before it broke again. Tried a second time to switch to the second pickaxe, but the same error repeated itself and I was able to mine 1 more block before the reappearing pickaxe broke again. This time I went to my chest and put some cobblestone in it before trying to go to the second pickaxe. This broke the cycle and I was able to use the fresh pickaxe to mine more blocks.
Seed:
Creative/Survival: has always been a survival world
Options chosen: No Explosion,No Fire (was made before to have bonus chest)
Does it happen every time you load that map?: Yes
Does it happen everywhere or just a certain location?:Certain Location bottom right corner of the map that turned into a snow biome
Is the map Offline/Online or Either when it happens.: offline and online, most of the time offline as I don't like trollers
My map was created, on the day of 1.7.3 (July 16th) I haven't been in the side of the map where it changed from a forest biome to a Snow Biome and it only does it in the snow biome if you go 1 block out of the snow biome it stops it, I have tried multiple things to get rid of it, at first thinking it was the ice from the ocean below (Im 29 blocks from the world limit) every time I enter the world it does it, but only in one section of the bottom right corner, it seems strange but I'm guessing its lagging because it went from being a complete jungle biome to a snow biome where the ocean is frozen (also tried waiting for the ocean to be completely covered see if it from lag, didn't work :().i've tried leaving the area, but its a big area to forget about, and will soon need to come back to it, so i've been trying to find a way to get rid of, or do I just need a new world and a new farm O_O, and this is my first post
it happens to me most when i first load up the map and try to use TNT, where the TNT explodes, but stays there. That and when i try to join up with my friend in Florida, but he has a lousy connection, so i dont know if it qualifies.
I had this happen to me recently, playing a single player map with BTW conversion. The bug seems to be localized around my first base, which I have recently decided to change the layout of a little. I have no idea what caused this bug, and at first thought it was lag, or memory issue - but after loading the save the next day - after a long rest for my laptop the bug is still effecting the save.
Version: 1.5.2 - BTW requirement
Seed: Illuminati (I doubt this is relevant, as I've played this seed several ttimes, and this is the first time I have even come across this bug
Survival Mode: I only ever play survival.

Does it happen every time? As I mentioned, it started only last night, seemingly at random. After a block was mined, it would drop, and the durability o a tool would drop. Not every block reappeared, and those which did would restore durability to the tool. A second mining of the block would often remove it.
I saved, thinking i was lag, and turned off the game. It's happening again today, though not as quickly, and blocks don't always drop. Tools however, are losing durability/breaking regardless. I'm hoping leaving the area and returning will fix the problem.
Does it happen everywhere or just a certain location? I have so far only checked the local area (where the bug started, at my base) but will look into other chunks.
You've posted this to the Xbox section, but I gather you are on PC. On the Xbox, 4J Studios looked into this bug and have suggested that it is indeed lag related.
