Wow I have never gotten my item back. Once it broke it broke. Nice and interesting video.The block part is exactly what happend to me, but without me getting back my item. I didnt go into the nether and come back though. Maybe I should have?
I've been doing a bit of testing on this, it seems to happen more when you're near the mob limit. Was building an exp farm, things went hay wire for a little and my NPC's were backing up big time. Needless to say when they were in full blown chaos this glitch would show itself. I clear the mobs its fine. They back up again and blamo im double/triple smacking every block. Hope this helps
Further to my post #22, I have had a bout of reappearing blocks again so the cleaning of my console and controllers did not prevent it from repeating, although it seems to have improved the frequency a bit. This occurence happened in offline single player in the tutorial world while using my cobblestone generator at my home base (beside the castle and animal pens). It was raining at the time. Blocks would briefly reappear and then eventually pop and drop on their own (but only after a noticeable delay). I had been playing for about an hour when it started, but I had saved and exited the game briefly within the 1/2 hour of it happening. I also went out and slaughtered a few animals just to see whether they would also do their death throws (jiggling for a moment or two when killed instead of just going poof). They did. So, I would say in my case at least, the two events appear to be triggered by the same thing... and my guess would be an overload lag induced initially by the rain starting up (also in an area of the map where there are several animals being kept).
I should also add that my cobblestone generator uses a piston, so I generally generate a line of 9 cobblestone and then mine the line all at once. I am not mining the cobblestone blocks as they initially appear from the lava. My cobblestone generator is located in the original tunnel between the main tutorial and the castle grounds.
We are currently investigating the cause of this bug.
I am looking to join any worlds you are currently seeing this issue in order to see it first hand.

I have found that only the SNOW area (and below) of my map has this issue. Once I get nearest to the snow, the blocks come back and not until I log out and come back, do they disappear.
Usually it only happens to me for like 20 seconds, then it goes away. play time before varies, and it's happened in multiple worlds that have been created after the patch. I haven't seen it in older worlds. I always play online when it happens.
I don't think you could join the game in progress to see it, though. it's a random occurrence.

Date Happened: 1/9/2013 Seed: Unsure Survival Original options, easy difficulty Online only, multiplayer GT: spawn3731 Time: 9:30 PM - 11:00 PM EST Hours: On this world, 15-20 Hours in total, on the day of, about 3-4 hours
To give a little more information, this happened last night/early this morning around 2:00 or 3:00 AM. The world is hosted by my friend and we play multiplayer, invite only, with 1 other friend. We were all exploring a cavern and suddenly all three of us experienced the blocks reappearing after mining them. I personally was mining a block of iron at the time. The blocks disappear, drop the block of iron, gives me the iron block, and then maybe after 2-3 seconds max, if not just a second flat, reappears. When mined again the block gives no iron, disappears, and then reappears in the said time again. We saved the game (which may have been a mistake) all exited and then loaded again, still happened.
We then left the game completely (to the xbox dashboard), I had everyone restart their internet modems to hopefully clear their connections thinking it was just some cluttered up information in the lines or something. We restarted the world, and it was still happening. We went up to ground level (above the cavern/outside) and tried to mine up there, same deal, blocks disappeared and reappeared. So we decided to make another game, I hosted and so far it is not doing this. However, 10-15 minutes before we went into the cavern and started mining, we had built a enchanting table and enchanted some items, before we did that, this had not been happening. We also have not tried mining in a section far away from that area other than the surface above the cavern, which is very deep where this happened. Have not tried that world since, but if asked we can do so. As for the time when we're on, for at least today and tomorrow we can probably be on at almost any time if you want to check it out.
Yes, tried Exit, Exit & Save, dashboarding and reloading, xbox restarting. sometimes it stops it for 10 minutes but often it goes right back to reappearing after the reload

online AND offline. also happens alone or with other players. also, it seems to vary sometimes who it happens to. they can be reappearing for someone playing with me and not me, or start for both at the exact same time.
ALSO. This only started happening about 3-4 days ago. I played for a month and a half before this even happened once. and when its not happening, it usually takes about 10-30 minutes before it starts for me.

Anyway, I've experienced this as well. I find it seems to have something to do with how much is happening within the currently generated chunks. It seems more activity eventually causes it. Whether it be pistons going nuts with minecarts doing circles around detector rails, craploads of passive mobs, Or perhaps just a game thats been going for a while and generated alot of the map.
I find it happens more often on maps when i'm nearby some of my machines such as constantly cycling pistons. I've also noticed that the rate of which it happens severely increases when playing on split screen. I've actually refrained from any split screen online until this is fixed. Its bad enough dealng with reappearing blocks online because of lag. I can't bear to think how horrible it would be with both lag and the bug.
I've also noticed that once it happens in my game, I can't kick people without my game freezing. It does it every time and it seems others are having the same issue
All of this occurs Offline in survival mode on at least easy as I haven't tested on anything else.
Also another thing I should note is that this bug seems closely related with the new sticky piston dupe, in which 2 blocks appear and one reappears after you break it, giving you an extra block. Fixing this bug might also fix the dupe.
I was talking to a buddy of mine who has been playing with a friend and yesterday this happened to them directly after creating and placing an enchantment table. So I think enchanting is the problem. Maybe you could start there and find the coding error in that section? Hope this helps a little bit more on this.
So for everyone else, just don't make or use enchanting tables.

I had been playing on it for a while. I moved from my house to a cave that I found. I had fallen and died then went back to set it up. Put in stairs etc. I found a dungeon towards the bottom and dug it out so as to create a spawn trap. I got some lava in a bucket, went back and then saved the game or exited and saved. When I went back in to the game 20 minutes later (and everytime ive tried after) I break a block I receive that block but it returns. If I try to break it again it does the animation of a breaking block and I receive no block. It then comes back like 2 seconds after or so.
I never used creative to cheat or anything like that. It was all me. Except for that stupid chest I asked for in the beginning that the game provides. Which is annoying now.
EDIT: I went in and the LT on blocks thing works ok. Still annoying. Also I noticed now that Lava once placed is invisible in spots and Water is invisible or does not flow correctly. I did manage to get one batch flowing but it then could not be removed and only half of it worked....Is there a thread for that?

I have a world me and buddy have played in and there is a particular area where we have noticed the reappearing bugs. We notice it everytime we play in the world online or off. The difference is though I've logged on his name to test it on my xbox. When doing so I get the reappearing problem way worse then I do when I'm joined to his game. No idea why. I have redstone "piston doors" that after opening manny of the blocks reappear where they started, and in addition the pistons usually dissappear(go invisible) left clicking doesnt really fix it since the pistons are hidden. I can left click and fix the blocks that reappeared but can't fix the invisible piston since the other blocks are in the way. Until I exit and reenter or leave area and return the pistons do not work, then upon returning they usually mess up with first attempt to use them. Found that redstone also returns to it's "previous" state... So this isn't limited to just blocks reappearing. The block completly returns visually to it's previous state. Some of this may have been metioned however I did not read through every post on this subject.
Now in this world I have lots of things powered by redstone overall. @ seperate minecart tracks, one for supplies and one for people. I have several train stations and the total track laid probably exceeds 1000. along with this I've made a system that every station has 4 to 5 minecarts backed up waiting to be "called" so they are sitting on a track somewhere in a row bumping into eachother. Not sure how much all the other things going on in the world might be causing the bug to start accuring.

We cleared out and flatened to standard see level 80 by 80 area for a castle. Bug started part way through doing the clearing. Not sure if that matters. I can recreate the event within minutes of loggin in everytime. If you need a world to join still to see bug in action let me know we are more then glad to assist.

Options chosen No Host or player options are activated, or ever have been. All player options are default. When it was first generated, it was set to peacefull for the first couple of days, but has since been played on Normal only.
Does it happen every time you load that map? It's gotten to be an evenuality when I play for awhile. Sometimes is comes & goes on it's own. Sometimes quitting & restarting will fix it for awhile - sometime that only helps for a few minutes. Sometimes it actually doesn't happen for a long time, but it always seems to happen at some point. Once it starts, Left clicking (left trigger) with a tool will make the blocks that have re-appeared disappear again most of the time. That gets tedious though. The blocks are fully solid prior to being removed by the left trigger though. I've been stuck inside them before & been pushed of of ledges as well (down an open shaft... into lava....) .

I've had it happen at my main home while removing blocks that I've placed myself, as well as while exploring & mining deep cave systems that are liteally accross the map from eachother (longer than the "on a rail" minecart track connecting them).

I always play with Friends can join activated. Most of the time the map is played solo or local co-op though & it happens in both cases.
For what it's also worth - I've had the "re-appearing enchanted tool" bug happen a bit in this world as well. This also means - I do have an enchanting table. I saw that was someone's theory, so figured I may as well include the info.
Edit - Side note (more info is always better than less) I tried clearing the cache yesterday in hopes that may help and that did not have any effect on the bug at all.

BUT i saw a video somewhere about MCXBLA and it was talking about the frame-rate staying at 60 or 50fps i believe. so I just thought about what if pistons had something to do with this, pistons usually glitch or disappear quickly when retracting or extending and they reappear again (the extended parts). maybe just from having them in the world leads to this block reappearing bug.
