I like starting in creative and getting a nice foundation down with a house, village, wheat farm, etc. Then I switch to survival and continue on from there. It's nice being able to fly around the seed first too, just in case the seed turns out to be total crap.
Depends on my mood. Some of my old worlds i am considering creative as well, but there is one that I refuse to do so since that is the one I worked with most, even if it was in peaceful...><
Most the time though I use creative to mainly doll up the villages, and/or fly around on a new world generated to look for a good map. Haven't really done a whole lot with creative being creative yet... More like modifying or adding to is more like it. I mainly prefer playing survival, even if it is on peaceful, just so I can work with the land. I dig that hole will be there, and may serve a purpose later on to spur an idea. If any difficulty I use easy, even if it really is no different from normal at the moment, just to mainly get mob loot and such.
Maybe one of these days i will actually see a new world generated that I would love to do creative in but right now i am not feeling it, or in the mood. Superflat does not hold my interest at all... No land to work will just flat as the eye can see.

My First World, always getting back to is a pleasure I enjoy with each new update that brings in more things to add in.
First I build a huge amazing house, then I store a crap load of diamond swords in a large chest,a crap load of 64 diamond blocks in a large chest, a crap load of diamond picks,axes,shovels,and hoes,a crap load of diamond armor,,a crap load of TNT with 1 flint and steel, a crap loaf of bows and arrows,a crap load of food, 1 cake for looking at it and more. Then, I make a pen and throw eggs at it, which makes chickens, then I find a pig and cage it, cause I love pigs then I get dogs, and THEN I switch to survival. Legit.
By the way, you kind of over-complicated the poll. This would've made a bit more sense:

Just something to keep in mind for the future. The multiple question "Yes or No" set up is a bit awkward to look at. Especially when looking at the results.
