You cannot leave the game, you have to get this one after starting the tutorial straight off.

Is it on the mushroom biom??

go threw the nether portal and search the nether fortress i found one in a chest there.

although i haven't found it myself i've heard the last one is in a nether fortress. don't know how to find the fortress yet...i got lost last time i was in the nether in the tutorial world.
