My biggest fail was when I was in a lave heavy area. I had one diamond and one dirt block and my inventory was nearly full so I decided to toss the dirt block.
Once I was out exploring in my world.It was night time and I stumbled across a zombie. I went up to it and started to attack it.I tried to 180 it,and it got lucky to kill me.I
was able to recover my items fortunately.This was on Normal difficulty.
I'm out of the habit now but I lost my map a good 10+ times by pressing B instead of LB/RB, I had to backtrack to find it as I wasn't familiar with minecraft.
My friend and I saw one of those weird floating sand things. It was covering some surface lava. I warned him of lava but he didnt listen so he punched the sand and he sank into lava. I was cracking up, he lost half of his supplies and ores.
I built a whole castle out of obsidian and decided I didnt like it :/ I had a lot of help and it still took about 15 hours solid with diamond pickaxes to destroy the thing.

I don't really have bad luck in minecraft. My buddy, though, holy crap! We have a few worlds we play together. First time we explored the nether together, he harvested about half of the glowstone in it and then managed to fall into lava. We had been tunnel mining before that too...so he lost probably 80-100 blocks of glowstone, (equivalent in dust, of course), 40 iron, 25 gold, a minecart, and some diamond. Now, that sucked. Hard. Just sunday he was holding most of our good materials in the new world we started. Not as much as the first time...thank the lord. But this time he was just digging sand up on a beach for glass. Nothing looked amiss about the beach. Suddenly, BAM, whole beach falls in and he gets wasted in a hidden lava chamber. I looked over at him, (we were split screened) and I said, "Dude, seriously? How is it even possible you manage to die in ALL THE LAVAZ?!!?"
I've thought hard about using him as a lava dowsing rod.

I would call this a "curse', not really a fail, but every time I have tried to mine diamonds, I get a good lot and then go for the last block and then BAM! lava! Freak out! hit the crouch button and then fall into lava! Happened four times in a row!
last one, my buddy and I are clearing a mountain with TNT. LOTS OF IT! He yells run for it! I lag for a second... You could figure out the rest.
Almost every story involves diamond, and lava, since everyone knows how they go, I won't mention it.
But on my favourite mine craft PC world, I decided to go adventuring, make a new map, explore a new area, you know, find some cool stuff.
So I start by going from my house directly north, go all the passed my initial map, bring out the new map, go from one side to the other.
Then I find this really cool area, perfect for a new house, start chopping wood, now normally when I go adventuring and I don't know exactly where I'm going I turn at 90 degree angles and count my steps, so that way its easy to remember how to get back.
Anyways, so I start building, go for a smoke break, and them come back, but now I don't remember which way I turned. So I guess, and I figure out pretty quick I guessed wrong, so I take another guess on how to get back, feeling confident that if I go directly north of the position I'am at, I'll make it there. Good 15 minutes (real time obviously) of walking I figure out I'm lost, rage quit, and that is the last time I played mine craft on PC.
That was actually right around when the 1.8 or 1.9 update came out on the PC, so thats why I'm so stoked for the xbox update, it'll feel like an entirely new game to me I'm sure.

I was exploring a huge cave system that was very successful. on my way back out with 10+ diamonds, d pick and shovel, iron armor and a bunch of iron and gold, i tried to jump over lava.... FAIL! haha
Me and my buddy had that happen to us.
He died like 15 times. (He's a lave magnet)
I died twice.
Then our save went corrupt and we had to escape the nether again.
one time my cousin and i decided to put the difficulty on hard and go into the nether. as soon as we spawn a ghast damages the portal and neither of us had flint and steel. and then i accidently hit a pigmen and short story made shorter, we died pathetically
I was being totally careless and got lost in a cave with a ton of gold and diamond so I had to kill my self or grow trees underground I am now living in a cave

Carrying 4 stacks of obsidian, 4 iron picks and 2 diamond picks (with autosave off). Fall into a single block of lava I'm using for lighting while I work. Think, "I can't afford to lose all that stuff," so exit without saving.
Start over, clear the same area, get down to the same point, fall in the same. damn. hole. carrying basically the same stuff. Think, "Well, I started over once already, so..." exit without saving.

Thats funny, i myself was building a large structure and i am a host to maps for my two oldest kids, i supply like everything they use because in the game i am a hoarder. well i was fring pigs for the cooked chops affect and i didnt pay attention to one of them running from me while on fire. needless to say, they burnt down half of my birchwood tree forest (300 trees). I was freaking out trying to put it out before all of them burnt down.
My 11 year old decided to build a lava structure to help defend his house against attack, well, he made a mistake and fell in it and died, carrying some diamonds, diamond tools, and diamond vest, a map, chops, torches, and blah blah blah. Pockets were pretty full, he did this on his island. noone was near him. So he respawn and the original spot. So half a day later of walking in the game, he made it back, stuff gone but with his mishap, half of his house burnt down(made out of wood), lost three chests of stuff, and got pretty disappointed. We now supervise or help in his lava creations to help with possible mistakes.

i was in 3rd person and crouching and not and then crouching again then boom a creeper came in front of me i had gold Armour and diamond Armour in my inventory and i was neer my chests so all of my diamonds blew up
ps: i had the diamond Armour in my inventory because i was wearing that but my friend thought gold was more rear he was laughing at me for the rest of the day
I was playing with my cousin and i go to pour a bucket of water over a lava lake out of nowhere he falls in and the lava above him turns to obsidian luckily i had all but 1 of 23 diamonds and we only lost his iron so it wasn't all that bad
One time I added a basement to a house I had built, but I guess I forgot to put torches in it. Next time i came back in to my house, as i was unloading wool I'd been gathering, Hisssss...BOOM!. Blew me up, my wool chest to smithereens, and a sizable chunk of wall/floor/ceiling from my house. Didn't lose any ore or diamonds, but i lost all my wool and 3 and half stacks of wood I was carrying.
