Mine I have in my basement, hidden by a wall, which if you press the lever it will open up the wall and there is the portal. Someone can easily grief though and close the wall behind you. which sucks.
i make a a piston somewhere in my room then a hidden button in my room that turns off the piston and opens a whole with a ladder leading way down. Then the room is a mixured of cobblestone and mossy stone.(A dungeon looking room made from the mossy stone from two dungeons)

I don't hide my Nether Portals, mainly because I don't build much in the Nether so I don't care what somebody does in there.
I've got one Nether portal in a room under a well, it's supposed to look like Luigi's Mansion but I can't get a painting to hide the portal.
Another one is just in my strip mine, placed for the heck of it, something I may need to remove.
Don't really know how to describe the last one so I'll just post a picture of it.
The Portal is in a small little outcropping to the right of the flag, there is only one way up to it which is a small little tunnel up to it. You can make out the little outcropping with the torch on the dirt wall, I think I can even catch a glimpse of the top of the Nether Portal

1. One nether portal I put in my basement, behind a wall that is easily filled back in.
2. Another is not really hidden but has a fountain of water around it (looks awesome!)
3. The third portal is hidden.... UP A TREE!
