Anyway I am a little of both mentioned.
Depends on what I am building/terraforming.
A building/structure in general I usually eye ball things, but will put framework down first; usually logs, and then add the walls & roof(if needed).
For landscape that is free form, as that is mainly eye balling trial and error. The except is trees, which take care in placement for bonemeal to grow them when needed. Sometimes having to knock down a bit of neighboring structure just to grow the tree, then rebuilding what portion I knocked down.
Coming up with a design or theme I want to build in my style. Is trial and error but with similar process to the first mentioned with building/structure. The only difference is I trial and error till I get roughly what I was looking for. This involves mix match of the many colors/blocks in designs as well how the initial framework is to be built. Also variations in how block placement can affect a view/angles.
When it comes to furniture or inside the buildings/structures, I have various items/furniture in mind; and that leads to trial & error of where to place and in some cases what size. (length of couch/table, etc..) So the only free hand is what a person would do in a real house when moving furniture around. lol
Quarry has its' own structured style to it. As I make a x by x square with wood scaffolding that is at the edge/border all around, stairs every so often to lead from level to level as I mine out downward. I always manage to build one of these, as I use a lot of stone bricks in the beginning of the game.
I don't usually use creative much, and do most in survival. Once I get a branch mine going I may stop using the quarry.
I usually gather resources in order to build, so farms I build with function and aesthetics in mind so a combination of terraforming and structures depending on what it is.
I know it was a simple question, but for me it is not really since it varies from creation to creation.:)

My First World, always getting back to is a pleasure I enjoy with each new update that brings in more things to add in.
