I get materials and make a starting house, which is usually more or less a cave house in a cliff. That way I have a night shelter. Then I can either mine in that cliff behind my place or get more materials to start building whatever I've planned on (an actual house, a town, etc.) and go back to that cave until my main house is complete.
Punching wood until I have a nice stack is always my very first thing, where I go from there depends on the landscape: if I can see a cave, I'll burn charcoal, make torches and a stone pickaxe/shovel, and get right to mining. If there's no cave, I'll put up a little shanty house, I'll make it out of dirt if I'm in the middle of a plains and there's not many trees around. I generally don't start building a house until I have enough materials to finish it to the end.
Before even making a new world I decide what I want to build: a giant statue to some dark god, a castle built right into the mountain, a floating island - whatever. After that I make a world either using a suitable seed or a random one. If I like what I see I proceed to explore the whole world, making note of any places of interest (lava, huge waterfalls, places for other statues/castles, deep caves). I do this on peaceful, obviously. After that it's time to build.
