Just like the title says what do you keep with you no matter what and why for me i have to have stones and flint and steel why i love to set things on fire and i love to build with stone
a pick axe (perferably diamond)
at least 1 stack of torches
a cooked pork chop
a mine cart
1 stack of dirt
1 bow with 1 stack of arrows
and 1 stack of wood
That's my usual ck list before leaving the house...
When mining:
-Stone pickaxe so I don't waste the others (I generally have 4+ on me when I go on a long mining run).
-Iron pickaxe for everything that needs it (too stingy to use iron on stone, coal, etc).
-Stack of sand (the falling property is really helpful to get down from high up).
-Stack of dirt (generally for making birdges).
-Bow & 1 stack of arrows
-Diamond sword
-Bucket of water
-1+ stacks of torches
It's a lot of gear but I hate not having what I need.
I really encourage carrying sand for two reasons:
1: You can fill in a lava pool instead of just covering the top layer. This is handy to prevent nasty surprises later on.
2: If you're high up you can make a pillar up to yourself and then dig it down by placing sand on the edge of the block you're standing on (while sneaking). This is super-handy when you find a drop-off in a cavern.

I just recently did some exploring of my map (i've been in my general spawn area since I purchased the game) and when I do that again I'll be sure to take a bed!
It depends on what I'm doing, but general rule of thumb is "don't take anything with you that you wouldn't want to lose, unless you really need it".
When I mine, I leave stuff like wood, coal, iron, spare torches, minecart and so forth in a chest in a central workspace area, then go mining with:
4x iron pickaxes (I generally find enough iron to make up for it, and it's faster)
2x stone spades (sometimes iron spades, but not a big deal)
a pile of torches
a bucket of water (for dealing with lava or putting myself out if I step in some)
an empty bucket (for getting rid of annoying lava or water sources I may stumble across)
I tend to do most of my mining on Peaceful mode though. If monsters were active, I'd add:
1x iron sword
1x bow
a pile of arrows
4x some kind of food, pork chops or mushroom stew generally. I have one in my quick access bar at all times.
I generally have some kind of armor on, iron seems to be the best for cost vs efficiency but it depends on how your personal resources are.
If I'm expecting to deal with obsidian then I'll pack a diamond pickaxe, of course, but I generally don't bother.
For overland missions, I'll add things like a stack of wood (to make a crafting table and other things on the fly), and perhaps an axe for gathering wood. I might carry more spades too as I'd expect to be digging through dirt and sand more than stone.

I find the gravity properties of sand to be helpful for that reason and also for making a temporary structure to get up high. If I ever want that pillar taken down it's easy by just mining the bottom block and letting it fall down one at a time.
I also like dirt and sand because you don't need a pickaxe to mine it. Mining with your hand isn't really THAT slow. Like you I'm stingy with my pickaxes and don't like to "waste" them on lessor materials.
Actually, I only take 15, 16 and 17 if I'm going "topside", I won't waste a diamond sword use on a chicken. If I'm clearing a cave and end up with a lot of ore, I just drop a chest and put the non-precious stuff like dirt and cobblestone in it.

Agreed, but I use Gravel instead of Sand. Gravel will actually turn into something useful (flint). This frees me up to use my sand for sandstone and glass.

This is all I need to make it on my own. These are the very essentials needed to stay safe, and anything else can be extracted form the world. My map seed is very open and flat, so navigation is a very easy issue. The pickaxe digs up the ground, the torches help me see, and the mushroom stew is a great safety precaution.
