First of all, I have an HDTV, split screen IS working. That is not my problem.
The problem is wrong user/controller is often assigned to the wrong part of the screen. As in I want to always be on the right side if two player, or upper right if 3-4 player. Frequently this wont happen and I have no idea how to change it or how to make sure I end up in the right spot in the first place. We either end up resetting the console until it's right (which is annoying) or switch controllers/avatars (which gets confusing).
There is probably a pattern to it, or a way to change it, but I have yet to figure it out. Does anyone know?
The person who launches the game in the console will always be on the top (left side if split vertically) as far as the other goes you just have to get in the game second or fourth if you have a 4 way split, I play split screen all the time and the order is as follow: First player (host) gets top left, the following player to get in to the game will be right side and so on, if you are all joining in at the same time, just check the little screen that pops right before you start the game, it should show who will be where and in that screen you can also decide if you want to trade spots with some one else by changing profiles right before the game starts.

(top left is p1, top right is p2, bot left is p3, bot right is p4) Which ever control presses a first will take the 1st open slot (one asking for "press a")

You have controllers 1, 2, and 3 plugged in.
Controller 1 is already signed in as your main profile (most likely as this is the one you turned your xbox on with)
Controller 2 is on with no profile signed in.
Controller 3 is on with no profile signed in.
From here controller 2 or 3 may press a.
If:
Controller 2 presses a 1st it will take the right side screen as player 2 (or top right if 3 or more total players)
(Or bottom screen if horizontal split-screen option is enabled... look in options)
Controller 3 presses a 1st it will take the right side screen as player 2 (or top right if 3 or more total players)
(Or bottom screen if horizontal split-screen option is enabled... look in options)

If you are trying to change the screens In-game you can only do this for players 2,3, and 4, as signing out of player 1 will end the game...
Here's what you do:
For example you wasnt to change player 3's screen possition with player 2.
Press the middle button on player 3 and then press x. (switch profile) You will be propted to sign out, select yes.
Repeat this process for player 2.
Then press the middle button on player 3's controller and sign in as the profile you wish to use as player 2 (screen 2 in MC)
Next press a to enter the game.
Then press the middle button on player 2's controller and sign in as the profile you wish to use as player 3 (screen 3 in MC)
Next press a to enter the game.
My XBox must be doing something funny. Because player1 was just assigned upper right. which is fine, it's my map. But I don't always end up there on my maps. At the time I launched the map my profile was the only one signed in.
So, still confusing.
*EDIT* Sometimes, rarely, before a map is launched I get a big white pop up for players to sign in on. Other times don't get that one and there is just the tiny grey press start to join. Doesn't seem to matter if other profiles are already signed in or not. I seem to be able to assign screen sections on the big white join game, while the tiny grey one has some system I don't understand.
How do I the big white pop up to show up consistently?

No guest accounts. All 3 accounts have gold memberships. We made use of the 2 months for $2 thingy they are doing right now for new subscribers, just so a friend could join us online.
You've been very helpful. Thanks to your suggestion I still figured out that I could enter a map, see which controller went to which screen sections. Leave map, sign out, swap controllers with each other, sign in with the controller that happened to be assigned to my screen section ---> Profit! Just a bit time consuming and a tangled mess of cords.

As long as all player are logged in to Live right before you launch the game you should get the 4 player sign in prompt box, it has to be a online game(server) if I'm not mistaking

...? That is strange... If It skips this screen just have the controller you want to be p2 press start before p3/p4.
What you can do is start up the game with only player one.
Then try to sign in player 2 by pressing start on the controller.
(If you are using a guest account for p2)
(If you are not using a guest account ignore these steps.)
The game is buggy here so do this.
Press start and it will ask you to select a profile. (HOWEVER NOT LISTING GUEST)
Back out of this list.
Then press the middle button on p2's controller and press x to sign in.
Select guest.
Press start on p2's controller and guest should now be listed, select it.
p2 will now join the game as guest account.
(warning guest accounts inventory is NEVER saved)
(use a chest if they wish to save items)
