There is a bug in the game where you start taking drowning damage while in water even though your air meter is filled all the way to the top! This caused me to drown, lose all my stuff, and rage quit. This is usually caused by going in water, surfacing right about when you are about to drown, then go straight back down again. Your air meter will be full, but you will be taking damage and most likely die if you were in deep enough water.
