I recently made something with the same idea as this, but I never made a minecraft forum thread :(. I have things like the need for water, rotting food, super-powered mobs, and a disease. Also, I figured out how to basically remove the lag from command block mechanisms such as mine. You can see the full list of features here: http://www.planetminecraft.com/project/apocalypse-survival-2/ I also suggest adding an execute command to the command blocks, at all players, and then add a radius tag to the original commands of lets say 100 so it only affects mobs in a 100 block radius.

EDIT: Got the disease without stepping on any mushrooms, cure doesn't work, and I still had the disease after dying.

No, that seems too overpowered.

No, that seems too overpowered.
it would happen only once per mob and it takes twice as long for them to drink it than it takes for you to drink a potion.
There was one mutation. A wither skeleton thing, knocked it out with a stone sword (In the 1st 20mins of gameplay)
Other than that, nothing changed.
In the first 20 mins would mean in the middle of the night. Do you mean to tell me that you got a stone sword, then went out in the middle of the night to kill a wither skeleton?
Was this a new world or an old world? The slowness and mining fatigue only happens to people who are low-level, and if you recently installed this in an older survival world... you get the picture. And did the thirst just not appear? Did it not increment?

Ohkay, I shall explain:
I put the schematic in my New World, got basic tools, and it said an mutation occurred, it so happened to spawn under the bedrock box redstone thing. And the thirst didn't come up on a scoreboard display, so I don't know if it incremented.
I'm going to download it again in a new world, and see if it works better.

Actually, you know what? I think it's because the main clock in the latest version doesn't work correctly and you have to start it yourself. I'll fix that in the next update.
In the meantime, I made a new, smaller vmod called Creative Helper, you should look it up.

All right, True Survivalism v1.0.2 has been released, and with it BUGFIXES!
Technical changes:
~Many fill-clock command blocks have been changed to player-radius non-hopper sequential clock command blocks, so to reduce lag and help debug.
~The center fill clock now does not activate until the dialog is complete. Note: Other non-fill clocks, such as poison death, mutations, and low-level debuffs, still are active during the dialog.
Planned changes:
>More custom potions
>Early-game snares and such to help you get meat
>New ways to get items only obtainable by shears

Don't give up! I promise you it is worth the effort. You already have quite a crowd around this thread so I decided to help out. I practically fixed lag for a superflat world with a few entities. However I don't guarantee that this will be a full resolution. I made some changes that seem minor but actually make a huge difference, and I left a book in your inventory to help you. Go ahead and turn this into a schematic and try it in a normal world, because the computer i happen to be using can't handle anything other than superflat.

This vmod is officially BACK IN DEVELOPMENT! I will be pushing out a new update soon that will fix most, if not all, the entity lag.

Can you get a world with the scematic in it? I have a mac and Mcedit is broken on my mac. I also couldn't do scematics when it did work.
I will work on that.

Yeah, that would be cool, i have ubuntu and i cannot get mcedit for it so that is the only way for me to play vanilla mods.
I will make a showcase world and a normal world. Currently, however, I'm switching my main focus to another vmod, MineMachines, that I'm making.

Well, scratch all this. Apparently the lag isn't fixed. Oh well.
