Officially? Almost never (Z Pigmen are the only one I can thing of). Unofficially (liking an idea but not giving any credit)? Almost never. Coincidentally (adding something that has been suggested but not because of a suggestion)? All the time. For example, polar bears have been suggested many times, but I doubt they were added directly because of a suggestion.
In fact I would wager that every single new addition in the last several updates at least have had a similar suggestion made beforehand on some platform or another, even if the developers did not see it. And yes, 99% of developers do not listen to external suggestions. Many that do only listen to the very broad strokes of suggestions if they listen at all.

Makes me wonder why there is a suggestions subreddit.

Of those posted on Reddit, these were implemented in game.
Surprisingly, lots of those are very useful additions that came from the ideas of the community!
I've got a suggestion that will be implemented in game some time and I'm so excited.

There are actually quite a lot of suggestions that were added to the game, from small things like dead bushes dropping sticks, to bigger suggestions like Ender Chests, and Elyta firework boosting.

I'm pretty sure 90-99% of all content in Minecraft were never suggested. Like polar bears instead of colored beds.
PS: is it common for game developers to rarely listen to suggestions?

Most of our suggestions get rejected by Mojang MC.
Wonder why.
Which one?
