I've got an issue with this texturepack where mobs faces turn white/gray at random. Changing texture pack prevents it.
Using shader mod and optifine. Is this something you believe could even be related to this?

I also have this same issue. I'll raise it on the Optifine Discord group, as they might have an idea what's causing it (I hope!)
[EDIT] Hmmm - I seem to have solved it, but not sure how! I have changed shaders pack (from Sildur's Enhanced to Builder's QOL), and also changed the brightness from "Bright" to "Moody" (as the QOL shaders is quite bright on it's own), and it isn't doing it now (touch wood).
