The items.png file contains the icon, next to the book. The actual map files are inside minecraft.jar/misc. They're called mapbg.png and mapicons.png.

As I said, the actual map files are inside minecraft.jar/misc. They're called mapbg.png and mapicons.png. You hold mapbg.png.
