I wouldn't include (I don't think they would either) Doku or John Smith in a "realistic" category myself. They are among the best, but more of an RGP game look than realistic.
I made Stankepack to be realistic. Not so dark themed as Mythos. It's not a complete pack as I didn't do most of the items sheet or mobs. But you can always use another mob pack and items sheet with it.

There is no complete non-doku, non-RPG, non-vanilla 32p pack that got my interest.
Johnsmith, not so RPG, but not complete.
Glimmar's Steampunk is very good, but not really what we can call "realistic".
I'm not very good at making graphics, but I'm working on doing a pack. Damn this is hard.
