Ok, maybe I found your problem. If you start the game and load a world the first time, Creeper always show skin 1. Only new spawned Creepers show biome-specific skins.
I discovered the same thing when I was investigating flohmannjr's problem. Fixed for next release.

I think the reason is Jukebox are TileEntities, and they only change their metadata fom 0 to 1 when activated, and 1 to 0 when deactivated. I have seen with NBTExplorer that there the value "Record" of this TileEntitie represent the id of actual disk
At the moment, CTM is completely unaware of TileEntities and their metadata. I'll need to do some research to see if this is feasible.
Optifine (Version for 1.4.2) ; Installed it after MCPatcher
Overwriting MCPatcher's changes with Optifine all but guarantees problems like this. In your case, MCPatcher adds a new field called randomMobsInfo to the EntityLiving class, but Optifine overwrites that file (ln.class) entirely. Last time I checked, only Better Skies was compatible with Optifine.

Turning off better skies worked. But now I am confused as to how to start working on a HD texturepack? I just started on a 16x16 and saw this that allows for HD and now I want to try that out instead.

I plan to look at the request for different CTM textures for each record on the jukebox next. It was too complicated to be ready in time for today's release, but I think it's doable and I hope to be able to add it in a future update.
I re-tested here and the problem persists. Tried both options (with and without the biomes line) and get the exact same results. After that I deleted all Minecraft files, download the game, patch with 2.4.3_03 again, test again, and same results.

The signs are hard to read, but it's River on the left and Ice Mountains on the right of the divider.
Try one more thing. Patch without Random Mobs and load up your world, then save and quit. This will flush any saved biome skins data. Patch with Random Mobs enabled again and see if that makes any difference.

I found the problem, Custom Colors. I didn't know this was necessary, I always patch with HD Textures, HD Font, Random Mobs and Connected Textures. I apologize to you and _zombiehunter, you guys working the problem and I derping around. Sorry.

It's working for me. Did you install ModLoader via the patcher's Add (+) button?

The first time installing an HD pack is always the hardest but soon as you get it done it's like second nature.
Thanls, but, how do I make an HD pack? I open the textures and they're in 16 by 16, and I can't make smaller pixels within those pixels.
Happened to me, and as someone said, turning off better skies or random mobs should help. I just turned off skies and I could play.
