I Think your having the same problem I had which is your computer doesn't have enough memory to load the night sky for whatever texture pack your using. The only thing that worked for me was to half the image size of the night sky and then it worked.

Yeah it might have to be the same aspect ratio I think. Because one of the night sky pictures on this texture pack I used was 3072 x 2048 and I downsized it by half which made it 1536 x 1024 and then it worked.

Try patching it without Better Glass. Mine wouldn't load until I took that out.
I also just updated to 1.4.2, with Better Skies and Better Grass turned off it's running fine for me. At first Skies was on and Grass was off, then I turned off Skies and it stopped going to a black screen.

It's choking on a modified EntityRenderer.java / ayt.class. I only post here because it worked on previous versions, and in trying to run it down, it dies no matter how small the change to EntityRenderer is. Vanilla EntityRenderer = fine. EntityRenderer with a single variable added inside a method (and never even referenced again) and the patcher dies with the log as above
Changes to RenderGlobal are picked up and patched around fine. Changes in EntityRenderer used to be, but now they are not, even when absolutely minimal.
Help! I tried it with and without the zombie texture compatibility fix and checked and the texture pack I am using is fine, but all the zombies look horrible! their head texture is on their legs. This didn't happen until I updated to 1.4.
So I am guessing there is a slight problem with the latest mcpatcher, correct ? I also tried to update it for 1.4 but my computer thinks it is a trojan and will not install.

Hey Guys, any confirmation about MCPatcher working on 1.4.2? I see the exe has been updated but I wonder if Random Mobs will work :S gotta test it and update meh post if I encounter bugs!
