Does anyone know of a pack that has successfully altered the pop-up item text?
Doesn't matter what the alteration is - "Wood" to "Logs", "Porkchops" to "Bacon", anything.
I know the 'lang' folder has a 'en_US.lang' file, with the correct labels for every inventory thing in the game.
What I DON'T know is how to make Minecraft use your file over its own.
And exactly, I've heard people say they figured it out, but am yet to find a working example...
I just did it with my personal pack. You don't add another en_US.lang you just modify the current one in notpad. Just REMEMBER to delete the META INF folder.
I have been trying to do the same thing. All you really have to do is obtain a en_US.lang file, (you can get this from your minecraft.jar) modify it, and them put it into your texture pack in a folder called "lang". that will cause Minecraft to replace the default en_US.lang with the one you modified. The .lang file is very straight forward, so it should't be too hard to change.
I found this in Toasted Baby's texture pack. As far as I know, this doesn't require any mods what so ever.
Hope this helps.

I believe that you also have to manually install your modded file in minscraft.jar. It's not supported from the texturepacks folder.
