This is probably a problem with the download. Try either to simply redownload the pack or open it and search for items.png in the gui folder in the zip file. The armor and the sticks for example should be in the top left corner. This part is probably empty in your case. I have no idea how this could happen but you should be able to fix it by redownloading the pack.
If this part isn't empty try reinstalling Minecraft the way Misa described it in the OP.

Update: I now know it is something to do with MCPatcher. When I select the texture pack all of the items show up perfectly fine, except then I have the flames on the corners of grass and no water. When I patch it though, I don't have items. So now I know it is MCPatcher's fault, but still don't know the exact problem or how to fix it.

What version of Minecraft are you running and what version of MCPatcher do you have installed? These two need to be in sync with each other to avoid the graphical glitches that you are experiencing. I've actually run into this problem myself, and the way to fix it is to update either MCPatcher, or Minecraft so the two versions match each other.

My Minecraft is 1.4.6 and MCPatcher is 2.4.5_01. Does patcher need to be 2.4.6? I click to download the latest one on the MCPatcher thread.
I love this texture pack, the problem is, it lags me. Oddly, the lag is only a one-two second lag. I don't have the problem with other texture packs such as PokeCraft or Faithful.

Getting sick after getting back from the trip put me out for a little longer than I had anticipated. Now it's time to sort through all the catch-up work I need to do.
My PM and email backlog is absolutely massive, but the texture pack has priority. Once I've gotten all caught up to date on my end with the pack I'll start answering all the PM's and emails that have been piling up and then I'll see what I can do about getting out an update for the latest snapshot 'n' such. After I'm on top of all of that, I'll start reading all the posts I missed on vacation and get around to doing a response post if necessary.

I just have a problem with carrots (every texture does this), if anyone looks you eat them from the GREEN end. Kinda gross XD
But I'm having aproblem, and it's just this pack. Not sure what's causing it, but the 3D sprites look messed up. Not all of them, just most. Some of them look totally perfect and others look... gross. Anyone here know what's causing it/how to fix? I'd be really grateful

actually i do you need to get the new optifine, the old version messed up the 3Dlines becuse they where making it compatible to stick out of the new frames but they messed it up and its been since fixed. just go get the newer one should fix you right up.
If that doesn't fix it then i'm sorry i even posted cause that would then be beyond me. :/
Sorry. She won't be doing support for GLSL which barely had any updates apart from compatibility with new versions of Minecraft, and SEUS which relies on Optifine which Misa has issues with. If you really want to use the shaders, you'll have to disable these special features.

Disable custom compass in mcpatcher.
What is that under in mcpatcher? I'm not finding it.
Tab "Options" -> uncheck "Enable custom compass texture"

Unfortunately, no. I believe there was a 32x version before 'til Misa went to 64x, which she will be sticking to.
BTW, Misa, if you read this, I hope you could somehow bring the alternative skin pack back. I miss that one.
