Textures can already be added while playing the game. All that's needed is a way to make the textures in game. Surely an ingame texture maker wouldn't be to difficult. A Gui for such a thing would probably look something like this. In steps you would:
1-Find a block you want to paint.
2-Right click the side you want to paint with some kind of tool.
3-A Gui would pop up with that side of the texture that you can paint pixel by pixel.
4-Paint
5-Save your design to the brush. Now you have a texture that can be painted on any block side that is the same shape and size. The textures would act very much like the carpenter's block's covers but look like the carpenter's blocks overlays.
-This is my first time making a mod suggestion so please be supportive and make suggestions to make this idea better. There is always room for improvement.

OpenBlocks has a way to paint blocks. You cannot draw it pixel-by-pixel however, instead you can put an image together from geometric shapes. Somewhat like banners.

I am aware that similar mods, like open blocks, exist. However they are not exactly what i'm looking for. I know that it is certainly possible to be able to paint blocks pixel by pixel, though it may not be easy to create such a mod.

Actually I'm not sure that it is possible to paint a block pixel by pixel, unless you create an entire design mod that adds a grid kind of like the block parts mod which lets you place pieces of blocks in certain positions. Basically they'd need to create a block overlay gui in Minecraft and that would take a lot of time and patience with the coding.

What if it worked similar to the chisel/tiles in carpenter's blocks? The main difference would be a Gui that lets players create the textures pixel by pixel in game. Hm, it would be difficult though to get it working server side using Java.
Difficult, but not impossible Nothing worth doing is ever easy or else everyone would do it
