So what exactly is it that a Mod creator / owner will not approve their mods from being inserted into a "ModPack"? They are typically ok with the end user visiting the main forum thread, downloading it themselves, and using it however they wish, but when it comes to someone wanting to use it in a ModPack, they have complete reservation from them doing so.
Is it because...
.. they want to track the download?
.. ModPack owners are annoying?
.. they feel it might not give them credit for their efforts?
.. they want the end user or player to be forced to visit the thread for current updates and chat?
.. they just dont want them to use it in a ModPack because their own personality?
.. of licensing?
.. of support?
I would assume most ModPacks are non-successful, reach a very limited user base but it seems like the Dev's see the ModPack creators as just an annoying, non-educated little twirp about to claim their rights to the mod.
Because they limit it from ModPacks, doesn't that create less interest in an SMP environment? Heaven forbid I walk all my players through installing it and supporting it when a ModPack resolves this.
And how are Mod creators tracking their mods when they approve its use in a pack? Do they keep a list usually?

I presume it feels like a loss of control, a loss of attention, and a compromise of one's creative vision.
Not, mind you, a loss of income; I think that would be far, far overstating the amount of money one can possibly get from adfly here. As far as I know, they pay less than a fraction of a cent per click.
