Either a mod, or they honestly do just have someone from the community step up to the plate and pay a few bucks to make a new account so that they can play the part of that character. Though, it's probably the former...

They have a whole staff. Yogscast also has enough money to buy Minecraft accounts. So if they need a new character, a staff member is most likely assigned the character and Simon or Lewis buys the account.. Easy as that. ^^
It honestly wouldn't be out of the question that they just find someone willing to shell out the money for a new account from their community to play the part of a new character. I wouldn't doubt it if you can rule out it being a plugin.

I own a serve rthe only name changer show this in chat: ~Honeydew
And shows their real name above their head.
1. Mod. It's VERY easy to set it up so that it sends your username as something different than it is.
2. Server Plugin. Many have a "nick name" ability that lets you set a custom name instead of your default.
3. Cheat Engine / Data Editing. Not sure it if works client side online, but I'm certain server side you can change names with data editing.
4. Alt account. Self Explanatory.
IIRC there is a hacked launcher out there that allows you to set any minecraft username you choose.
Of course since it is a hacked launcher you can only connect to servers that don't check with the login servers.
I remember being directed to one once when I signed up for a minor role in a machinima.

The Yogscast buys all of the accounts. Last I heard from one of their podcasts they said they've spent somewhere around 120 pounds on accounts so far but that podcast was before BBQ Bay.
They're youtube partners, and they have a job.
Youtube money goes to making youtube videos, buying accounts, etc., paychecks from their job go to IRL things.
