You'll need to first get Minecraft Forge. Then Install Flan's mod and run the game so a "Flan" folder will be created in your directory (.minecraft folder, by default), that's where you put the pack files.

The texture needs actual shading, its all one color Just now realizing you only asked for the model and not the texture
