EDIT: Could you make it so that when you die, you view falls to the side, just like your player? Not only that, but it always annoyed me that my head keeps bobbing up and down when I die in vanilla. Lastly, could you make it so that you see other players die instead of just disappear?
I found a bug between Gulliver and this mod. It causes, that you see the inner of your head that is normally not displayed. Same happens with the Better Animations Collection mod.
Here a example:

Well, I could see if I can make the camera follow the player's body as it falls over, but I'm not sure how well I could make it work.

I don't know, I can't really find out without my home computer. The power at my house is out because of a storm, and it won't be back till Wednesday. The Forge logs would be a good place to look for a list, but I'm not sure if it has a good contiguous list in it. (it might just have the mod initialization output)
Do you use a launcher? All I really need is a list of mods you installed to minecraft.jar.

Turn off IFP mode and then disable the GUI.
Did you install the RenderPlayer API? Did you install Kes's utils?

I was able to find the mod that is causing the issue, it's the C98Core mod from this mod author:

Do you have Animated Player, or some other mod that makes your player model different in any way? Like More Player Models, or that one Voxel box mod?

These are the current mods I have:
Optifine
Mob Amputation
Mob Dismemberment
Zan's Minimap
And then the improved first person mod.
Does that help?
Oh yeah and I also have minecapes.
