It's part of Minecraft 1.5, not 1.5.1. First make sure you have the latest version of Mithril. I tested with Optifine B3, both Standard and Ultra, using manual install. I haven't tested MCPatcher. And I tested with Backpacks. It works for me. I haven't tested all the other mods you listed.

Ok. The only thing I can think of is that this machine is old and running Win7 32bit and something there doesn't jive. Here is exactly what I did.
Minecraft installed fresh, everything. Even moved my saves and texture packs.
The only thing in the folder is Minecraft.exe
Downloaded and ran fine.
Downloaded a fresh copy of "minecraftforge-universal-1.5.1-7.7.1.611.zip" Manually installed. Deleted "META-INF"
Loaded MC and created a random world just fine.
Downloaded a fresh copy of "OptiFine_1.5.1_HD_U_B3.zip" Manually installed.
Loaded MC and created a random world.
Downloaded "Mithril.zip" from page 1 of this thread.
Copied "bronze_1&2.png" and "mithril_1&2.png" to the armor folder in minecraft.jar
Dropped Mithril.zip into the Mods folder
Executed Minecraft and it hangs a split second after the MOJANG splash screen.
Same error it looks like.

You shouldn't call that "old". I volunteer at the Computer Lending Library. They get computers from government departments and crown corporations (companies owned by government). When they upgrade, they donate their old computers. Low income people (many on welfare) can take a one week course in the basics of computers and Office programs. It's free, but they have to attend all day every day for a week. If they complete it they get a computer, a free 2 year loan. And if they break it, they can bring it back any Friday for a free repair. I'm the one who does the repair. The hope is they'll learn skills that let them get a job. Until last week we were still giving out Windows XP. Last Friday was the first issue of Windows 7.
And the computer I have is also Windows XP Professional 32-bit. Although a lot more powerful: Core2 Duo, 4GB of RAM (3.5GB usable), and my video card is GeForce 8800 Ultra. My server is running Linux Server; command line only, no GUI.

One more thing. You said you created a world, then installed the mod. If you do that then any ores or crops from the mod will not generate; at least not near your spawn point. Only chunks generated after you install the mod will have them.

Very odd, I downloaded the Mithril.zip from the first link on page 1 about 3 times. It had only 1 folder named "Mithril" I believe. It had all the textures in it. Then all the class files were outside the folder.
The one I DL'd from your post above has 3 folders. "Armor" "Mithril" and "Mods"
Didn't change much, just plant generators. They're now independant of biome name.
Grape vines never were dependent on biome. They require grass, and an adjacent vertical surface. Growth requires a surface to grow on, and light level 9.
Hop vines also never were dependant on biome. They require grass and a vertical surface. This update makes hops require visible sky for full sunlight. Growth conditions haven't changed since the initial post for Booze.
Heather is the only significant change. It used to require biome name "Extreme Hills" to generate. Now it requires:
biome maxHight >= 0.8
biome temperature <= 0.9
biome rainfall between 0.3 and 0.5
Note: for vanilla Minecraft this allows heather to generate in Extreme Hills Edge.
Added Icebox. This works like a chest, but a large icebox is two stacked vertically. If you put 2 side-by-side, they don't merge but their handles do align toward each other. You can place an icebox directly beside a chest or cupboard. So if you want a wall of storage, you can alternate between large cupboards and iceboxes.
Crafting recipe requires 4 iron ingots, 2 planks, 2 tall grass (for insulation), and one block of ice. To harvest tall grass, right-click with sheers:
Place 2 iceboxes, one on top of the other, to make a large icebox. Shown is a double large icebox; that's a total of 4 iceboxes. And of course you can craft an icebox with a tripwire to make a trapped icebox.
Mithril was converted to Forge months ago. For the current version of Minecraft it ONLY works on Forge. And the other mods were specifically written for Forge.

I started writing mods to prove to employers that I can write in Java. Last summer I had a job opportunity that required experience with the Delphi programming language, DB2 database, IBM mainframe stuff, as well as web stuff including HTML, CSS, JavaScript, mySQL database, and Java. I had experience with everything but Java. For a programmer with as many years experience as I, with over a dozen different computer languages including Visual C++ with MFC and JavaScript, learning Java is trivial. But they didn't hire me. So I started writing mods for Minecraft as a demonstration that I can code in Java. But they still didn't hire me. There are other jobs that require Java, so I continue. The purpose is to get my name known.
With that goal, I need full credit for all my mods. For mods to be included in someone else's pack defeats that purpose. Even if you include credit somewhere, it'll be hidden where no one will ever read it. And others could copy my code from your pack and delete all references to my name. So no, I don't think it's a good idea to put my mods in a pack. If you want to include a link to my mods, please do.
As someone mentioned, there's a texture missing for Churn. This was a block I hadn't implemented yet, so now it's here. The churn will change milk into butter. Right-click to open like any custom furnace, place a bucket of milk in the bottom slot, and an empty wooden bowl in the top slot. There isn't any fuel, just wait for the progress bar to finish. The bucket will become an empty bucket as soon as the progress bar starts. When it finishes the bowl will move to the output slot, it will be filled with butter.
In real life one gallon of whole milk makes about 1/3 pound of butter, depending on breed of cow. So think of a bucket as one gallon, and a bowl of butter as 1/3 pound. I had intended the churn handle to go up and down while the progress bar is moving, but I need this finished quick. I should focus on other features. I may add that later. For now, you get butter. Yes, "butter" is declared through oredictionary.

Just wanted to let you know that with a little re-arranging of the contents of the booze_1-5-1.zip I was able to use the mod WITH the textures, without having to manually install the mod.

I have uploaded the .zip file whenever I posted an update. I don't know why my ISP doesn't serve the current file. There must be a cache somewhere.
I'm planning to create a website for my mods. I have a web server sitting in my home office, but got permission to set it up where I volunteer. I got the new server to work there, so I will replace 4 servers, each 5U height, with 2 servers, each 1U height. That will give me 18U of room in the rack. My web server is 7U height. I got all the SCSI RAID drives to work (had to replace one) and started installation of Windows Server 2003R2 Standard. It will run Windows Server Update Services to cache updates from Microsoft for internal use there. I have a license for Windows Server 2008, but everything where I volunteer is older equipment. Besides, 2003R2 will run WSUS, and doesn't require as much memory. The other server has the latest version of Linux Ubuntu Server, acts as our file server as well as Clonezilla Server for deployment. I already have my web server running, but only have a residential high speed internet connection for my home office. My ISP really doesn't like people to run a web server over a residential internet line. If I made it live here, my ISP would cut off my internet. But I can get it going where I volunteer. That means I'll have complete control of the server. So updates will be published as soon as I upload them.
Right now, everything I said is "updated" has already been uploaded. It's just a question fo when my ISP will feel like making those available.
Update Today (April 20), I moved the .zip file for the current version of all my updates. Instead of being hosted by the website from my ISP, it's on a club website that I maintain. That club isn't Minecraft, but I'm president of the local chapter so I can. That web server crashed a couple months ago, but they got it going again. Most importantly we don't have the cashe problem. The new server I'm trying to set up is delayed. (Long story, not going to get into it.) Bottom line: you can download the latest update for Kitchens.

There may very well be a cache somewhere. I thought I was just going crazy tho! Reading back through this whole thread, it seems as though this has happened at least once before. Looking forward to DLing the latest versions (was having issues getting Mithril to load -- but I have other issues as well... notably with enhanced portals conflicting with slab/stair craft conflicting with forgottennature... argh!

How would that work? Vines are actually a block. They render in what appears to be the "air" block beside the solid block. They render a pixels in from the face of the air block, so appear in front of the solid block they "climb". The problem with a fence is it doesn't render on the face of a block, it's in the middle. So there would be significant space between the vines and the fence. Same issue with cobblestone walls, or iron bars.
How about a 'trellis' block that renders at the front edge of the boundary?

but it reminds me of when I was writing mods for ES4 and FO3... all of our 'new item' ID's were prefixed with our handle to avoid such conflict... apparently this train of thought hasn't translated over to FML yet.

I feel so out done. At least I have countertops. He doesn't.
Forge does prefix most objects with mod name. However, when you register a Block or TileEntity it isn't. And of course the name published with OreDictionary isn't.
I changed all those names from Fridge to Icebox. If someone wants to add other languages, the localization string is also changed. Now it's "kitchens.icebox" for a single block, or "kitchens.iceboxDouble" for a double block.
I also updated Mithril. Grape vines were allowed to dangle down, but dangling grape vines were not rendered. Now they are.
::I just installed his mod. Mr.Crayfish's fridge has coils in the back. But doors do not animate, no sound effects, and cabinets have a smaller inventory than a small chest. His has 3 rows of 5 colums, while my small cupboard is the same as a small chest: 3 rows of 9 columns. And my large cupboard is the same as a large chest: 6 rows of 9 columns. So I could argue my mod is more practical for survival mode. And I'm not sure what the furnace-like interface of his fridge is for. Can you freeze stuff? Turn water into ice? And crafting his fridge requires 7 blocks of iron, a chest, and a furnace. Crafting an icebox requires 4 ingots of iron, 2 planks, 2 tall grass, and 1 block of ice. His cabinet requires a chest plus 8 planks, while a cupboard requires 8 planks, just like a chest. So again, I would say mine is better for survival mode. Still, his other stuff is nice. I'm going to play with it.
::Wellll, there's another incompatiblity. In the Mithril mod, in order to get a furnace spew toxic gas, I had to replace it. The new furnace has the same name, texture, crafting recipe, and works exactly the same. The only difference is when you smelt tennantite ore or twisted bronze ingots, it produces toxic gas. But the new furnace also has a different block ID. That means any mod that requires a furnace in a recipe won't accept the new furnace. Oops.
TBH I prefer your fridge to his, which is why I decided to rename his fridge instead of yours.
I'm not entirely sure about the whole furnace-like bit, I haven't messed around with it too much yet.
Thanks for changing your itemnames though, i'll grab the updated versions and let you know how that turned out.

having a bit of an issue every once in a while with the salt you added to the worldgen, just when i try to generate a new world SOMETIMES. whether it's a new MC world, or a new age in mystcraft, once in a while the salt kicks up an error in worldgen.

I think something you have is looking for Minecraft 1.5 instead of 1.5.1.
Ok. The only thing I can think of is that this machine is old and running Win7 32bit and something there doesn't jive. Here is exactly what I did.
Minecraft installed fresh, everything. Even moved my saves and texture packs.
The only thing in the folder is Minecraft.exe
Downloaded and ran fine.
Downloaded a fresh copy of "minecraftforge-universal-1.5.1-7.7.1.611.zip" Manually installed. Deleted "META-INF"
Loaded MC and created a random world just fine.
Downloaded a fresh copy of "OptiFine_1.5.1_HD_U_B3.zip" Manually installed.
Loaded MC and created a random world.
Downloaded "Mithril.zip" from page 1 of this thread.
Copied "bronze_1&2.png" and "mithril_1&2.png" to the armor folder in minecraft.jar
Dropped Mithril.zip into the Mods folder
Executed Minecraft and it hangs a split second after the MOJANG splash screen.
Same error it looks like.

You shouldn't call that "old". I volunteer at the Computer Lending Library. They get computers from government departments and crown corporations (companies owned by government). When they upgrade, they donate their old computers. Low income people (many on welfare) can take a one week course in the basics of computers and Office programs. It's free, but they have to attend all day every day for a week. If they complete it they get a computer, a free 2 year loan. And if they break it, they can bring it back any Friday for a free repair. I'm the one who does the repair. The hope is they'll learn skills that let them get a job. Until last week we were still giving out Windows XP. Last Friday was the first issue of Windows 7.
And the computer I have is also Windows XP Professional 32-bit. Although a lot more powerful: Core2 Duo, 4GB of RAM (3.5GB usable), and my video card is GeForce 8800 Ultra. My server is running Linux Server; command line only, no GUI.

Very odd, I downloaded the Mithril.zip from the first link on page 1 about 3 times. It had only 1 folder named "Mithril" I believe. It had all the textures in it. Then all the class files were outside the folder.
The one I DL'd from your post above has 3 folders. "Armor" "Mithril" and "Mods"
And it works!

Didn't change much, just plant generators. They're now independant of biome name.
Grape vines never were dependent on biome. They require grass, and an adjacent vertical surface. Growth requires a surface to grow on, and light level 9.
Hop vines also never were dependant on biome. They require grass and a vertical surface. This update makes hops require visible sky for full sunlight. Growth conditions haven't changed since the initial post for Booze.
Heather is the only significant change. It used to require biome name "Extreme Hills" to generate. Now it requires:
Added Icebox. This works like a chest, but a large icebox is two stacked vertically. If you put 2 side-by-side, they don't merge but their handles do align toward each other. You can place an icebox directly beside a chest or cupboard. So if you want a wall of storage, you can alternate between large cupboards and iceboxes.
Crafting recipe requires 4 iron ingots, 2 planks, 2 tall grass (for insulation), and one block of ice. To harvest tall grass, right-click with sheers:
Place 2 iceboxes, one on top of the other, to make a large icebox. Shown is a double large icebox; that's a total of 4 iceboxes. And of course you can craft an icebox with a tripwire to make a trapped icebox.
Mithril was converted to Forge months ago. For the current version of Minecraft it ONLY works on Forge. And the other mods were specifically written for Forge.
I started writing mods to prove to employers that I can write in Java. Last summer I had a job opportunity that required experience with the Delphi programming language, DB2 database, IBM mainframe stuff, as well as web stuff including HTML, CSS, JavaScript, mySQL database, and Java. I had experience with everything but Java. For a programmer with as many years experience as I, with over a dozen different computer languages including Visual C++ with MFC and JavaScript, learning Java is trivial. But they didn't hire me. So I started writing mods for Minecraft as a demonstration that I can code in Java. But they still didn't hire me. There are other jobs that require Java, so I continue. The purpose is to get my name known.
With that goal, I need full credit for all my mods. For mods to be included in someone else's pack defeats that purpose. Even if you include credit somewhere, it'll be hidden where no one will ever read it. And others could copy my code from your pack and delete all references to my name. So no, I don't think it's a good idea to put my mods in a pack. If you want to include a link to my mods, please do.
As someone mentioned, there's a texture missing for Churn. This was a block I hadn't implemented yet, so now it's here. The churn will change milk into butter. Right-click to open like any custom furnace, place a bucket of milk in the bottom slot, and an empty wooden bowl in the top slot. There isn't any fuel, just wait for the progress bar to finish. The bucket will become an empty bucket as soon as the progress bar starts. When it finishes the bowl will move to the output slot, it will be filled with butter.
In real life one gallon of whole milk makes about 1/3 pound of butter, depending on breed of cow. So think of a bucket as one gallon, and a bowl of butter as 1/3 pound. I had intended the churn handle to go up and down while the progress bar is moving, but I need this finished quick. I should focus on other features. I may add that later. For now, you get butter. Yes, "butter" is declared through oredictionary.

I'm planning to create a website for my mods. I have a web server sitting in my home office, but got permission to set it up where I volunteer. I got the new server to work there, so I will replace 4 servers, each 5U height, with 2 servers, each 1U height. That will give me 18U of room in the rack. My web server is 7U height. I got all the SCSI RAID drives to work (had to replace one) and started installation of Windows Server 2003R2 Standard. It will run Windows Server Update Services to cache updates from Microsoft for internal use there. I have a license for Windows Server 2008, but everything where I volunteer is older equipment. Besides, 2003R2 will run WSUS, and doesn't require as much memory. The other server has the latest version of Linux Ubuntu Server, acts as our file server as well as Clonezilla Server for deployment. I already have my web server running, but only have a residential high speed internet connection for my home office. My ISP really doesn't like people to run a web server over a residential internet line. If I made it live here, my ISP would cut off my internet. But I can get it going where I volunteer. That means I'll have complete control of the server. So updates will be published as soon as I upload them.
Right now, everything I said is "updated" has already been uploaded. It's just a question fo when my ISP will feel like making those available.
Update Today (April 20), I moved the .zip file for the current version of all my updates. Instead of being hosted by the website from my ISP, it's on a club website that I maintain. That club isn't Minecraft, but I'm president of the local chapter so I can. That web server crashed a couple months ago, but they got it going again. Most importantly we don't have the cashe problem. The new server I'm trying to set up is delayed. (Long story, not going to get into it.) Bottom line: you can download the latest update for Kitchens.
There may very well be a cache somewhere. I thought I was just going crazy tho! Reading back through this whole thread, it seems as though this has happened at least once before. Looking forward to DLing the latest versions (was having issues getting Mithril to load -- but I have other issues as well... notably with enhanced portals conflicting with slab/stair craft conflicting with forgottennature... argh!

How would that work? Vines are actually a block. They render in what appears to be the "air" block beside the solid block. They render a pixels in from the face of the air block, so appear in front of the solid block they "climb". The problem with a fence is it doesn't render on the face of a block, it's in the middle. So there would be significant space between the vines and the fence. Same issue with cobblestone walls, or iron bars.

Forge does prefix most objects with mod name. However, when you register a Block or TileEntity it isn't. And of course the name published with OreDictionary isn't.
I changed all those names from Fridge to Icebox. If someone wants to add other languages, the localization string is also changed. Now it's "kitchens.icebox" for a single block, or "kitchens.iceboxDouble" for a double block.
I also updated Mithril. Grape vines were allowed to dangle down, but dangling grape vines were not rendered. Now they are.
::I just installed his mod. Mr.Crayfish's fridge has coils in the back. But doors do not animate, no sound effects, and cabinets have a smaller inventory than a small chest. His has 3 rows of 5 colums, while my small cupboard is the same as a small chest: 3 rows of 9 columns. And my large cupboard is the same as a large chest: 6 rows of 9 columns. So I could argue my mod is more practical for survival mode. And I'm not sure what the furnace-like interface of his fridge is for. Can you freeze stuff? Turn water into ice? And crafting his fridge requires 7 blocks of iron, a chest, and a furnace. Crafting an icebox requires 4 ingots of iron, 2 planks, 2 tall grass, and 1 block of ice. His cabinet requires a chest plus 8 planks, while a cupboard requires 8 planks, just like a chest. So again, I would say mine is better for survival mode. Still, his other stuff is nice. I'm going to play with it.
::Wellll, there's another incompatiblity. In the Mithril mod, in order to get a furnace spew toxic gas, I had to replace it. The new furnace has the same name, texture, crafting recipe, and works exactly the same. The only difference is when you smelt tennantite ore or twisted bronze ingots, it produces toxic gas. But the new furnace also has a different block ID. That means any mod that requires a furnace in a recipe won't accept the new furnace. Oops.
I'm not entirely sure about the whole furnace-like bit, I haven't messed around with it too much yet.
Thanks for changing your itemnames though, i'll grab the updated versions and let you know how that turned out.
I really need to get back into modding, so I can be of more help tracking these bugs down and just being helpful in general.

having a bit of an issue every once in a while with the salt you added to the worldgen, just when i try to generate a new world SOMETIMES. whether it's a new MC world, or a new age in mystcraft, once in a while the salt kicks up an error in worldgen.
