Shaders files *.fsh *.vsh are not included.
You may get these code from old GLSL Mod version or from daxnitro's wiki or from Sonic Ether's Unbelievable Shaders.
For manual install
1. Open minecraft.jar in WinRAR or 7-Zip or similar program
2. Delete META-INF
3. If you use Forge, copy files from Forge universal zip into minecraft.jar
4. Copy files from OptiFine zip into minecraft.jar
5. Copy files from ShadersMod into minecraft.jar
6. Copy *.fsh *.vsh files into minecraft.jar/shaders (or leave it empty and copy *.fsh *.vsh into .minecraft/shaders folder instead)
For use with Magic Launcher
2. If you use Forge, add Forge universal zip first
3. Add OptiFine zip below Forge universal
4. Add ShadersMod zip below OptiFine
5. Add shaders zip file (which has *.fsh *.vsh in shaders folder inside zip)
updated last 23/12/12. i will keep this post updated and link to here.
Vondoomcraft glsl shader support.
DO NOT POST ERROR LOGS HERE.
NEW UPDATE: FINAL - NOW SUPPORTS CTM features: most detailed terrain_s i have seen with specular sand, reflective iron, gold, diamond, lapis, emerald, obsidian, some ores ,all metal parts, bumpmap. shader includes realistic water, waving lava, grass, fire, flowers, vines, lilypads, wheat and leaves, godrays, lens flare. if you know how you can also turn on bokeh dof and motion-blur
NEW instructions: USE THESE VERSIONS. for minecraft 1.4.6
1. install optifine HD U A2
2. install Karyonix's glsl mod v1.7 only from here
3. delete meta-inf.
4. create shader folder in minecraft.jar
5. drag set1 or set2 of shaders into shader folder (only files inside shaders.zip inside set1, set2 folder).
6. delete entire ctm folder
7. drag texture files into vdc.
8. play and enjoy!
set1 shader file will NOT work for everyone. it is the better option. (they work for me but not doom)
set2 shader is for those who cannot use set1. (better compatibility- missing minor features)
cons: bumpmap only works in certain light conditions.best time is sunrise/sunset. intensive shaders will lower your fps to the point of being unplayable. if you have low fps try running in a window, change optifine settings, 720p etc... or run minecraft with more memory (i have a .bat file if people need it) also the end is stuffed i am trying to fix...
credit to original shaders mod: daxnitro and sonic ether
credit to improved glsl mod: karyonix
credit to bumpmap/reflection textures and compilation: zomb69
credit for testing: emperor doom till 5 in the morning! thanks doom

Having consistent and regular crashes with this shader mod. Wasn't sure if it was the mod, or the shaders I was using, or the other mods installed that were causing it. What I've been able to narrow it down to is the fact that it says "Expanding Tessellator buffer:" a few times in my console before the crash. That must be whats causing it. Anyone have a better idea whats actually happening?
EDIT: looks like its only happening if I run at 1080p. At the default resolution that buffer never expands and it runs fine. Problem is I have given minecraft 3GB and my video card has 2GB on it... not sure where this buffer is being held but I don't understand why it overflows like that.
EDIT AGAIN: Nvm, still crashes as a little window.
