I was just about to report the same bug. Nice to know that I'm not the only one getting the same crash. This crash as far as I can tell only effect the Wisp it doesn't happen when I look at an Angery Zombie or any of the Golems it only crashes when looking at the Wisp. My guess is that the mod can't figure out how to render the Wisp into the mob display area.
A few mod entities added to the default adjustment if you have them installed(mostly thaumcraft)
The portrait is now on a timer(roughly 3 seconds) when you stop looking at a mob, and will close when timed out. This can be adjusted from the main config. Set to -1 for the old behavior.
I'm running forge #411 and the portraits, names, and bars show up, but the health bar does not drop as mobs take damage. Oh also not seeing the scrolling numbers on hits.

Changing the position of the portrait will be coming in a future version, as soon as I feel like wrapping my head around the math. It's a problem because of screen scaling and inverse render direction of 2d gui vs 3d models, and I haven't thought of a magic formula yet, but I am working on it.
Edit: I am unable to recreate problems on version 411, Send me a message with all the mods you are using along with your logs.
Edit2: Found the magic formula, the next version will have the ability to move the portrait. Now I just have to think of how I will implement the feature. Done You guys will love it.(Will be in the next release)
Portrait preview backdrop is now much more attractive.
You can now re-position the portrait information within the game by pressing the Period Key(default). This key can be changed in settings, and is saved and read from the config for easier modpack customization.
After you press the period key, you can then grab and drag the display where ever you want. It will automatically correct itself if the gui scale or resolution changes, so don't worry about losing it.
Is there a way to only see the damage you make, i dont want to have the mob showing up in the left corner. I find it annoying, I would like to know if there's a way to disable that. so i can only see the damage.

Yes, check the config(DamangeIndicatorsMod.cfg), in the category mouse over health. Set Enabled to false.
Found the problem, fixing it now.
Should fix issues with wisp rendering.
Baby's are recognized now and rendering for babies can be adjusted from the configs.
Edit: DamageIndicators[1.4.5]v2.0.2.1 hotfix released so most(possibly all) mod creatures are detected for config adjustment.
Currently it disappears if you get far enough away, but yes, I will add a timer. I'll add a setting in the config so you can adjust it as well.

Which is why it is called a 'preview' ;), it already does that, and I will update the bukkit wrapper too at release.
Now shows potion status effects.
Bukkit server wrapper is now threaded(required for the new feature). So should have better performance as well as support for the new features.
