Er.. I have a few ideas for it too, but im not telling you that they aren't hard and ambicious because they are.
What about adding a infection chance when fighting Vampires and werewolves (about 10% per hit taken?). And when infected, the player change to a werewolf form at night (temporary skin change?) and get some advantages and disadvantages as well, like strengh, speed, but needs to eat more raw meat to keep the hunger bar up. (raw=4 slots refill, cooked= 1-2?).
As for a vampire, add a thirst bar that is filled when right clicking animals.
As for cure, equiping trinkets or drinking something... i leave that to you =D
