Which version are you using? Did you put both of the files into the minecraft.jar file?

Your cape is setup for your account correctly which means you're just doing something wrong while installing the mod. Are you sure that you're copying both files over?
Migrating your account won't affect your cape. What is your Minecraft username?

Create a PNG image that has the dimensions 22x17 and you can upload it to MCCapes.com to use it. If you want an example you can see Notch's cape here: http://s3.amazonaws.com/MinecraftCloaks/Notch.png
I deleted META-INF file and put the mod into the minecraft.jar.

Looks like your cape is working fine on the server side. Perhaps try installing the mod with MCPatcher?
If it uses the same files it won't be compatible. However you can use MCPatcher to install the mod and it will work with Forge.
