The 2D items not showing in items frames with fancy graphics is a Forge bug, not related to OptiFine.
I see that the main post says the version for 1.5 is compatible with Forge #598. I can't find any references to that kind of a number when searching for Forge. The most recent version on minecraftforge.net says Forge is version 7.7.0 for Minecraft 1.5. Where does one find these three digit numbered versions?
The Forge #XXX number is the build number. It uniquely identifies each Forge build (release).
The Forge 7.7.0 has 36 releases #559 to #661 (http://files.minecraftforge.net/). They have different features, different bugs and may be built for different Minecraft versions (1.5 and 1.5.1).
I was troubleshooting another mod when i noticed an Optifine "issue". I'm not sure if it's an actual issue, but here's the part that is bothering me and i'll post the whole log afterward it.

Sorry if I am bugging but I do wanna ask why you put the optifine updates to 1.5.1 as a preview... Doesn't it basically not notify people of an update? Or is it a way so it shows that it isn't 100% ready? Please answer and thank you.

However people who are more involved with the Optifine mod can often come to the thread and find previews or beta versions of a future update. This is how the versions are fleshed out, just like a minecraft snapshot. So when the "I want optifine NAO" Children of the world finally get what they want, they don't immediately start chanting. "FML! It's broken I want a new one!"

Ok, so I got a new graphics card and everything works fine except that in Minecraft multiplayer, when a block update occurs, the chunk that it occurred in disappears for a few seconds. I tried uninstalling Optifine and the issue went away. It happened on both 1.5 and 1.5.1.
Just tested A7 with Battered Old Stuff. Connected textures looking good.
It looks like the Biomes and renderPass options for connected textures aren't implemented, yet. Are these options in the "sooner" or "later" category?
CTM RenderPass is Later, CTM Biomes may be added soon.
Anyone know how to get the John Smith Legacy texture pack (http://www.jslegacy.com/) to work with this? After installing Optifine all I get is "Missing Texture" on everything. Without Optifine the John Smith Legacy texture pack works but looks pretty bad.

Only problem is it says run MCPatcher (or whatever it's called) which as far as I can tell doesn't exist anymore. Optifine is the other option besides MCPatcher but it doesn't work either
