It's pretty balanced for the most part. The advancing difficulty of the mobs correlates well with the increasing strength of the armor you find, although I notice this isn't the case with weapons. In the first two or three game days I found a Mithril sword with damage 6 and a 25% speed boost (which I just now noticed acquired Mending somewhere along the way. Huh. I wondered why its durability was going up), and although I have also found an Adamantite sword with damage 7, and a ...shovel... with damage 8.5, the loss of the speed boost, and the slowness of the attack speed on the shovel hasn't made me want to switch to either one. So basically I've been using the same weapon the entire game (I'm on day 61). It's a fine weapon, but a little more variety might be nice. I don't know, though. Maybe I wasn't supposed to find it as quickly as I did.
The only time I really had trouble was in the mine north of Al Khalid. Under the suspended walkway mobs seem to spawn in larger numbers than elsewhere. Since that's where one gets a quest item, it can't be avoided. I actually wound up putting the game in peaceful when I was down there, because the mobs wouldn't leave me alone long enough to pick up what I needed.
Other than that, like I said, it's been good. The minibosses at the end of the quests are difficult enough to be a challenge (maybe die once or twice, but nothing major), but not so difficult as to be frustrating or game-breaking. I've died 84 times, of which maybe a dozen or so are teleport errors (either I forget to add the minus sign in front of the first number, or I misjudge the y-coordinate) resulting in falling out of the world.
I do think the players might benefit from maps of Al Khalid and Avarrocka. In both those cities I often end up lost and simply wander around until I magically come across what I'm looking for. I've done all the the museum quests in Avarrocka, but I couldn't tell you where the museum is. "Somewhere in the north part" is about the best I could come up with.
I'm really enjoying this map, and look forward to the next installment. I've never played Runescape and can't compare this to that, but if they're at all alike I think I'd really enjoy it. Excellent work!
(So, the infinite-money bug I mentioned up above has just set my game back to the end of Tax Collector. Can I have the command for the end of Paragons of Misthalin, please?)

In that case, run the command /setblock -547 50 1186 redstone_block
hopefully the latest version we are releasing has fixed every issue

I'm having trouble with the "Tribes of Saragorgak" quest, i looked and for whatever reason the command is looking for a kill count of 20 bandits?
although the quest prerequisites have little to do with bandits and i have no access to killing any. It is fixed easily by replacing the command block with redstone but i thought you should know Edit: Still having trouble with the quest, the dialogue with the tribe leader simply returns humans do not belong here

There's a hump of dirt with a cave in it, near the water shrine (which is for a future quest. Don't worry about it now)

Hello i am exepriencing issues with the quest Tax collector i collected the money from all the diffrent people(yes i did look for the chest in the forest and i found it) but i returned to capitan rivon but he didn't say anything help me please
go to your downloads then find the gilenor 4th age folder unzip it open it open the file inside then copy that folder in to your minecraft saves folder
