Then, you're introduced to the "Energy Rod" and to the "Teleportation Device" (in Minecraft terms, "Fishing Rod" and "Ender Pearl"). After a lot of tests, you're redirected, again, to the normal (and abandoned) chambers, after meet GLaDOS in her reconstructed old lair. In some point, GLaDOS have the idea of recycle the "Energy Rod" and the "Teleportation Device". So, she moves you to new chambers she made. After some tests, Wheatley (who previously have send pre-recorded messages for you and GLaDOS) fall on the Earth again, right in Aperture. So, GLaDOS makes him responsible to fix the destroyed chambers. After all this, you back to the Relaxation Vault, waiting for new tests.

Yes, it uses. That's why I used version 1.4.4v0.1 instead of 1.4.5. I was planning to do with 1.4.5 or 1.4.6, but there's not much possibilities of tests without them

Don't worry, the PortalGun version 1.4.4v0.1 works perfectly with Minecraft 1.4.5. So if you're using this version of Minecraft, you need only change the .zip file in .minecraft/mods !!!

After meet GLaDOS, you have to jump into the incinerator (pressing the button inside that compartment at GLaDOS' left). Then you place portals inside the wall and in the incinerator, climb the stairs, press the red button on the wall and back to the chambers. What's the part where you said that was the last chamber? Is it still in the "Old Aperture" chambers? Because the end is that Relaxation Vault (the last screenshot at spoiler)... Anyway, thanks for the feedback. Right now I'm making another map, but to the QuantumCraft mod
EDIT: I think I've discovered what you mean. After Sector D-18 (in the Old Aperture chambers), you stop at a room in office style. Just keep going after there, in Sector D-19. After finish the last chamber of Sector D-19, you pass trough the door, stop into a room in office style again and, when you pass again trough the door, there's a big hole impeding you to continue, right? If that's the case, just fall into the hole and keep going, but if isn't, tell me please

English isn't my first language (and I refuse to use Google Translator, because it isn't so good for English translations), so it would probably have some spelling errors, but thanks for telling me! I'll give a look into the map and see what I can do . Also, I know that some peoples don't like downgrading, but the main reason I let the map in 1.4.5 is because this version has the "Thermal Discouragement Beam" (or Deadly Laser), and, without them, we would have less possibilities of tests, making the "feel in-game" much less outstanding. But, again, thanks for the feedback!

Strange...Maybe this happened because you've installed another version of the PortalGun mod, then deleted it to install the version 1.4.4v0.1. Anyway, you'll need to delete the "config/PortalGun.cfg" file and place "PortalGun.properties" (came together with the main download) in ".minecraft/config". The actual version of PortalGun have some changes in Portalgun.properties (and they changed to PortalGun.cfg), but in 1.4.4v0.1 this newer version doesn't work. I hope I helped you
