Did anyone notice he didn't drown or starve to death? Is there falling damage in that deep of water? Is there a command he could have used for insta-death? Was there a land mob floating about behind him? Or is there a new hostile water mob?
Maybe it's not likely, I'm only speculating.

mobs 1.5 x harder than hard mode
drop no items on death
bed sets spawn for only next death
max 1 diamond per chunk
You die, you get banned. Simple.
