Look closely. He is not holding it. The chest and the sand nearby are on a different level to the actual ground level. The sand he is 'holding' is also above the next level of sand.
I KNOW!!! The thing behind the cactus is a sign. He probably changed the textures or made it look different somehow. Also if that's not it then its probably that thing all the way in the back that looks like a cactus with a chunk bitten off of it.
the 2 changes are so obvious.
1- you can have sand pressure plates
2- if you set of 1 tnt, all the ones touching it get set of at the same time creating 1 big explosion (instead of many) destroying a beautiful desert sunset and a opened chest... wait what?!? YOU CAN OPEN CHEST LIDS?!?

The thing that people are thinking is a pressure plate or a block in a hand IS NOT. It has 18 pixels across the top, and a block of sand only has 16, which removes the idea of holding a block of sand.
The cacti are not horizontally neighbouring. There is a spot between them, but they are further back. If you are talking about the two cacti that are 2 blocks tall and infront of the "unknown item (more info below)", they aren't. The cactus behind them is between them, but two blocks back.
The block behind the cacti IS infact a chest. It is down in a dip, if you would look. Because it is in a dip, it looks smaller. It is also further away, which makes it look more pixelated and "2-dimentional".
There are your answers. Now stop saying its left-handed or a dead shrub, I'm tired of it.
You can only see the flag, not the pole. Directly adjacent to the cacti.
