So, the Nether. For reasons probably lagg related and making-it-work-with-older-worlds related, we never got Anvil for the Nether. They just made mobs not spawn on bedrock anymore as a fix, and now the top is littered with the occasional Ghast and shrooms. (Shrooms were actually removed in the upcoming 1.4.4, but anyways.).
Making it work with older worlds is probably possible, but my main thing is... now that we have Grumm on the rendering engine and Dinnerbone on the lighting, computers could probably, pound for pound, handle a 256 tall anvil. (We can handle CTM maps, after all.) So do you think this will ever be changed out, or will we always be unhappily graced with our tiny Nether?
The nether already uses Anvil for the level format, and it is already 256 blocks tall.
The Generator for "Hell" Biomes is presumably what would need to be changed to use More vertical space.

Yeah it would be nice if it was larger. Though I think one has to wonder how such an expansion should be done. Really there are two ways.
1. Stacked. This one would be relatively simple- just use the normal generator and stack the results. It would probably require some sort of tweak to the seed that is used (so they aren't exactly the same). Then simply not have a bedrock layer between them, perhaps with extra cave generating in the netherrack between them.
2. Expand the generation to be similar to what we already have, but simply expanded vertically; this would essentially mean more "shelves" and other structures, but going higher up.
Personally I'd think the second is better. I guess the question is, should this be the <only> thing they do when they do change the nether generator? I think it could be part of a grander project, IMO, Even with some of the more recent, awesome changes to the nether, such as nether forts, wither skeletons, blazes, etcetera, which give a good reason to explore the nether- there is still something I've always felt it was missing. All that netherrack, and no reason to ever dig through it- no loot or treasure to be found. A lot of the "nether expansion" threads really go overboard on this; All I personally think it needs are a few ores(even the same ones we already have with a netherrack base texture) and something akin to dungeons.
Wouldn't you get something like if you are on the bedrock ceiling of the current nether and are looking towards the anvil nether wouldn't you get a giant wall of nether stretching above the bedrock?
I'm thinking there isn't much point unless they added new generation content. I think what I would like to see in the nether that they could perhaps add to the upper 128 is another somewhat open area that is fairly flat and densely covered in lava lakes with a 'forest' of large mushrooms. Have quite a bit of soul sand patches too, perhaps even modify soul sand to where large mushrooms can be grown on them.
It would be nice, but if you have experimented with generator mods like Wedge you'll know that the current render engine gets super choppy and unstable when handling massive terrain on most computers. I think options for a bigger Nether or Overworld should come around after they finish the render rewrite. And even then it should be something you tick a box for when generating a world, much like how Large Biomes and Super Flat work.

Well, re-writing rendering and lighting is definitely going to improve things.
