Jeb mentioned somewhere on his twitter that the one stronghold thing was a bug, planning on adding more strongholds per world when bosses get added (if they do).
I don't think the map "knows" where the 3 are beforehand, so you don't have to explore the whole world to "find" them. Instead it's more like
1. Spawn chunk
2. Check: are there 3 strongholds in the world?
If no: 0.01% to make one.
If yes: 0% to make one.
I don't know this for sure, but it makes more sense.
I would also assume this is how it works. It's going to generate them as it does new chunks, and when it reaches the cap of three it stops making new ones.
