Thank you for drawing my attention to the "The Basics" section. Your 2 and 3 are obviously incorrect: Indeed, the fact me and others like Kenny are encountering this problem attests to the fact 2 is incorrect. Also (though I think you already know this) portals are only spawned in the opposite dimension on using a portal, not when lighting them. You should probably update the original post to reflect this.

I think you meant to say Nether portals here, that makes more sense. And it's true. I'm guessing the search range is currently 128 (closest power of 2 to the testing results), and 128 * 8 = 1024 which is why you have to walk an absurdly large distance to have the game auto-spawn a portal correctly (Going Earth to Nether).
The portal search in the nether needs to be 1/8 the search range on the overworld to account for the compression ratio (eg only searches nearest 16 blocks in the Nether, the way it was originally before the first patch). I have put this up on the bug tracker site, but it sadly seems to be being ignored... =(
I'll remove the word activate from my 2 and 3. It was intended to imply stepping into the portal, but "use" is good enough by itself.
And i agree that the surface-to-Nether search needs to be 1/8th of the nether-to-surface search to help resolve the minimal distance issue.
The problem with that, however, is along with ignoring the safe relocation, it ignores the 8x compression completely. So you'll come out at exactly the same co-ordinate position you went in.

Then after you dig yourself out, go back through the portal you came through, and then back again.

What portal you came through? "Boating" through a portal doesn't generate a portal on the opposite side, and if you mean the Overworld portal, it'll still link to where it "should" regardless of the exploit.
Now that I've started playing with portals, this guide will be very helpful.
Here's my experience: I created a portal and it made a two-way link with a nether-portal. So far, so good.
But then I died in the nether and I guessed it would create another topside portal (i.e. in the normal world) corresponding to where I died, as mentioned several times in this thread.
Shortly after, I entered and exited the Nether. I had collected some buckets of magma in order to create another portal in the Nether. After turning the magma into obsidian, I looked around a little more carefully and realised that the portal I had exited had corners, and my glass castle was not visible. It was very disconcerting to suddenly discover I wasn't where I thought I was. Fortunately I soon noticed my castle visible on the horizon.
I quickly discovered that there was a two-way link between this automatically-created portal and the nether portal, while my own created portal led to the nether portal. Before I had died, there was a two-way link between my created portal and the nether portal.
I guess that when the nether portal was created, it wasn't in quite the right place due to an obstruction, but it was close enough for two-way travel. But when I died, not far from the nether portal, it created a new topside portal in a location which was closer to the correct place than the original topside portal, so it changed the nether portal to link to it instead.
Soon after, I used the obsidian I had created to make a third topside portal near my spawn. Again it couldn't create a portal in the right place, because it decided the nether portal that already existed was close enough and used it again. So now I have 3 topside portals leading to 1 nether portal.
I shall fix them some time, but since I'm building a new cloud base not far from the two-way topside portal, it's handy to be able to travel from spawn to there by going via a single nether portal. Because I haven't become completely lost yet, it's kinda fun to be unaware of where a new portal will take me.

This is a very helpful guide. Now I know something new: Portals don't scale up or down in the nether, so building one at the bottom of a cave there and one at the top doesn't place your second portal underground.
Of course, this didn't help me figure out my first portal issue, right from when the halloween update came out.
Build one portal. Went in. Freaked out, left via same portal. Somehow ended up in a cave about 10 blocks down, 40 or so blocks away from my first entrance. Odd, but no problem in the end. Closed that portal and ran away.
In the end, same issue as mentioned in the OP, but I didn't know at the time.

Remember that the check for existing portals comes [b]before[/b] the check that the ideal location is valid. Your 3rd portal may well be capable of leading to a new position, but has just chosen to link to an existing portal anyway.
