The way I see it, you could have another hot key config in the options that toggles the duel wielding, default is T, You could press T anytime to use duel wield. I think the scroll wheel would still work perfectly fine, seeing as it scrolls for the item of choice. To pick what to duel wield in the second hand can be used by the P key, for Pick. It's hard to explain my concept but It could be done.

Same reason people place lights on the wall or ceiling in real life instead of holding them. Holding them takes up valuable arm space. Also, particular to Minecraft, it's not going to prevent a dark room from spawning skeletons to simply walk in with a torch from time to time. You'd still want to place them.
But I suspect that dynamic lighting like that -- that has to change every time the player moves -- might be very demanding on the game. And for a relative little gain. Unless it's not, 'cause I'm not really that computer-oriented. If it's not, sure. If it is, not worth it.
Glowstone can be pushed by pistons, and the sun moves in the sky (light levels change when it moves) so I think the lighting is already somewhat dynamic. But you could be right.
But as long as the glowstone is being pushed it is not glowstone but block 36. And block 36 doesn't give off light. So basically the light source disappears and appears at another place. And during sunset and sunrise the light level changes all over the surface almost at the same time. So both of your examples cannot really be compared with moving light sources light handheld torches. There was a mod actually that made torches produce light in the player's hand. But the problem was it lagged quite a lot and the light updated separately for every chunk. So a chunk only got light while you were in this particular chunk.
Ah. Well, think about this: other games have dynamic lighting, so I feel that perhaps minecraft could too. However, due to my limited knowledge of how graphics engines work, I don't know how easy or difficult this would be.

Frankly I'm surprised sometimes that Minecraft runs at all.
