I can't leave a tree sitting on the edge of a one dirt block high ledge, I'll either chop it down, or rebuild the ledge so there is grass on every side of the tree.
If I see anything, I mean ANYTHING, made out of dirt that is not a farm, the ground, or something that has a direct purpose, I will destroy it. On one server some guy made his house out of dirt near mine. I warned him several times to fix his house and he didn't. Finally I used 16 TNT blocks (has to be an even number), and blew the house up and then filled it in. Not that it really mattered. The guy was the owners cousin.... Let's just say I didn't stay on the server for very much longer.
My hot bar (from left to right) must be in the order of how big the item appears in my inventory. Every word on a sign must be capitalized. I can never have a non-full stack of something to the left of the corresponding item in a full stack. Everything has to be symmetrical. I can never have 2 torches beside each other in a cave. I must mine EVERY ore. I can never have more than 2 of the same tool. I won't upgrade a tool to a higher tier until the tool of the current tier is completely broken. I can't wear gold armor. I can't have 2 naturally spawned cacti diagonal from each other. In a superflat world, I can't dig down at all. And finally, no empty buckets in my inventory.
I don't see how you can call any of this OCD. OCD is when you take so many showers that your skin starts to fall off, or you can't touch anything out of fear of getting germs.
Real OCD is definitely something much worse than "Minecraft OCD" which is really just a bit of pop culture misusing a medical term. Real OCD causes debilitating effects based on recurring thoughts. "Minecraft OCD" is is more of a playing style than something that debilitates the player. I really wish more people would try to tell the difference.

My "Minecraft OCD" is as follows:
*My hotbar has a sword, pickaxe, shovel, torches, food in that order. Anything else that makes it's way into my inventory fills the other slots. I rarely carry a bow because I like having my weapon right next to my pick. If I do carry a bow, then I'm usually not holding a sword in my hotbar.
*When mining downwards, I'll fill in any ore I mine with cobblestone or stone (More often than not, it's cobblestone)
*My torch placement has to be evenly spaced out (Honestly, most people do this, not out of OCD but just because people intristically like patterns)
*Fill in creeper craters
*My "Minecraft OCD" thing that comes closest to real OCD is my recurring obsession with my original world which got corrupted in 1.8. My compulsion is to start a new world within a day or 2 of starting a new world. I don't really like starting new worlds, but any world I generate ends up losing my interest.

Also I sometimes like to keep misc blocks, tools and misc materials in my inventory to different places in my inventory. I also really hate having a full inventory, and whenever it is full I throw away multiple items that I'm not using at the moment that are easy to get. (String, saplings, mushrooms, glass etc.)
As for inventory? Mine always goes Sword, Bow if applicable, Pick Axe, Torches, Axe, whatever building blocks, then a stack of food on the end. I like to organize chests as well such as log, plank, slab, stairs, stick, etc
As for other things, not really OCD, I never use diamond tools. I'll make them but I will always stick with Iron as I never know when I may die or come upon some diamond again. This applies mainly on online servers when mines are mined out just about. I'll create a pick axe and enchant it but only use it long enough to get obsidian. Then it goes to my chest forever.

The trees MUST be cut down all the way o_o! I also always have my inventory the same way when mining . With arrows in the slot above the bow. The fact that everything is square helps my perfectionism.
I can't have 2 used items of the same thing in my inventory. I can't stand it. i.e. 1 used sword 1 almost broken sword. If I do then I use the most damaged one, not the other. I like preserving things, if that makes sense.
I have to have my hotbar near empty, and I cannot have a full inventory. My hotbar always has to be: Primary(sword), Auxiliary(torch, shears, etc.), then Secondary(Bow). Always. It provides a comfortable distance between my weapons, while leaving space for breathing room.

I love using bows, but decided to leave it out of my example...
I also always replant trees, NO floating trees from me!
