I have learned a lot on common and not-so common mistakes made by server owners / OP / Mods.
Good defenses to have:
*1 - Lockable and undestroyable chests are a must. Griefers use special xray mods to see chests regardless of were they are, and go after them first.
*2 - Anti fly detection in survival mode
*3 - Anti " fast mining " detection
*4 - Limit player actions to 2 per second. ( placing blocks & destroying blocks ). Advanced griefers use mass destruction / mass placement programs.
*5 - Good block logging - Being able to undo one person's actions is a must. There are ways around roll backs.
*6 - Disallow new folks to a server ( 60 days ) from using fire, lava, water, tnt, nd destroying other peoples blocks
*7 - Mcban - those that are serial griefers have many accounts, banning one just makes them log in using another one. The more accounts they are not allowed to use, the better.
*8 - White listing - griefing teams are very common, logging in and doing as much damage as possible in a short amount of time.
*9 - De-white listing - griefing teams are known to sign up to a white listed server long in advance of when they plan on attacking. Removing folks from a white list if they do not sign in at least once a week is recommended.
*10 - Zoning - Folks under x amount of days are not allowed to do anything outside of a certain area.
*11 - Update as often as possible. In one video aVo posted, they used a known exploit to gain mod rights, ruining the entire map, and crashing the server.
Problems:
*2: Sprinting gets messed up.
*3: If you're mining with a diamond pickaxe, you're screwed.
*4: If you're digging dirt with a diamond shovel, you're screwed.
*6: 60 days? Why not a week instead?
I wish I could grief the griefers so badly! We should do something like "Griefed? Get griefed." type of thing...
i did it on terraria. server i play on getting griefed and admin were sleeping.
i built almost all the fortress city and structures, so i was being protective about my creation.
without the super power of op. i just rapidly placing hellstone and hell brick around the spawn.
griefers got their butt roasted real nice.
and the next day admin read the log and promoted me to op xD
but yea, you cant really prevent griefers. theyll always find a new way to get around the system.
though, its safer to play on a smaller size white list server. admin knows the player, and all that.

I agree completely with the last part, 60 days is quite harsh
This isn't about plugins but store items in FURNACES. They don't have a mod for finding them (yet) and you can store 2 stacks of whatever. Not as efficient as chests, but which is less efficient- Chests w. more storage that get greifed daily or furnaces w. less storage that remain unfound except by the user or by luck?
If you're a pro greifer you'll understand that most of this doesnt work. I can write my own undetectable fly mod and even if the server has anti fly I can fly because my camera doesn't have the coordinates and speed, my player entity does. Saplings can prevent rollback/big brother. Ice+Silk touch is an easy way for water, and theres multiple ways of trail blazing people. These little tricks dont hurt greifers..
To the people saying "we should grief griefers", you don't even understand how griefers work. Griefers don't invest anything in servers; they just want to **** them up. The guy who built the castle isn't going to bomb your dirt hut for shits and giggles. The person with the dirt hut is going to dismantle the castle brick by brick. Lets say you actually did "grief" a griefer by destroying his base with TNT... well all you really did was make a big crater on YOUR OWN server.
This is why aVo is successful. Because SMP players are idiots.

The sole reason I don't like MC Bans is because idiots abuse it. My friend was framed and he can't even go on certain servers because of the jerks who run them. I don't grief. I'm strictly against it.

and i say that simply because we have found it useful on the server I play on, if a new player has a few logged bans, we still let him play,
and on that particular server, mods are required to post pictures of any ban made. with BB proof for griefing, and screenshot proof of anything else they ban for [I.E. swearing, hacking]
I love the fact almost no one knows how tools work. All tools swing at the same speed. The damage they do to blocks is different.
Speed hacking makes it possible to punch out an obsidian block almost instantly, due to the fact the hack makes your character hit the block hundreds of times a second.
