Pretty much I am wanting to remove the heart containers from chests and then re-add them myself with Custom Chest Loot to make sure they balance with the rest of the items I am tweaking. Although to be fair, I have yet to see one in a chest and I checked quite a few.

Ah, I had no option for turning just heart containers off w/o removing them from the game. I'm not sure if the multiplier will work when set to 0, but it's worth a shot.
--Also my loot isn't added to every chest, just newly generated chests (minecraft only generates chests in new chunks, so I add my loot in when the dungeons generate in new areas/lands). That's probably why you havn't seen any.

And I doubt it was that since I was doing creative tests on new worlds mostly, so it should have been in a newly generated world when I was checking. My guess is it was either bad luck or that other mods increased the total weights for the chest loot to the point the hearts were less common. Maybe both.

Hmm, as in you installed my mod first, then started a new world?
Yah, it has been active for a while. I been testing a bunch of mods, so a lot of new worlds have been created. I haven't checked too many chests, but I still find it a bit odd I haven't seen one besides spawned ones and a boss drop. So it may be the other mods, like simple ores, which add new items to chest rather commonly making the hearts even rarer. Later on, I plan to make the total weight over 1000 probably, so chances are any items not added in my custom config would become very rare.

If I might make a suggestion? Custom Loot Chest might be able to make the item you want to appear more frequently do so. It lets you configure what ends up in these chests, so if you want a higher chance of the heart items appearing, add more weight to them or add in multiple entries of the same item. I use this to weed out the garbage dungeon/SH chest loot I get (14 flippin' crowbars - c'mon man!) and in some cases works for chests added by mods as well (for example, Mariculture's Ocean Floor chests work, but Village Taverns is still w0nk). I hope this helps.
I'm already using it =P How experienced are you with configuring CCL? Because I usually prefer to disable any mods that add items to chests and manually add them with CCL myself to avoid any unwanted appearances; although I assumed that if I increase the total weight to 1,000-10,000 those items would nearly vanish as well. Still those rare encounters are something I'd rather not have.
And I just noticed some mod items are appearing in the default CCL configs... Well, I feel a tad silly... However, I did notice that Heart Containers are not appearing in the configs at all; only the vanilla and Simple Ores 2 items are. Do they appear in your config? Because I was under the impression that mods that add items directly to chests (usually the dungeon chest since many mods clone that one for their own structures), which would mean they would not show up in the config or be detected by CCL. But it seems for at least Simple Ores 2 this is not true...
Edit: And as soon as I delete my Custom Chest Loot configs, I reload to find a ton of new stuff appeared!

Tah dah, the missing heart containers -.- Well I feel silly. Apparently because I installed Custom Chest Loot before a bunch of other mods, it was blocking them. My guess is that the mods applies their loot first, and the CCL copied it and sorta overwrote it with its copy. I'll have to talk to Toast about this because this behavior is new to me...

Now that I think about it, I no longer just add to chests manually, but use the weights system.
My implementation of chest loot is rather simplistic currently. I havn't done much research into weights, and I think I created the weights to be decently common for vanilla minecraft+ more health.
I believe my weights are 6*multiplier for one heart container and 8*multiplier for 1-3 heart pieces.
(For a normal dungeon chest, the other types of chests have slightly varied rates)
I don't think I considered the affect of many mods with chest loots. Do you think this is the issue?
Configs can be tricky, especially with multiple mods adding items. If the total weight does increase drastically, players could just use the multiplier to make them more common. Personally, I think the default weights may be a tad high, at least for running without other mods; but that is a matter of preference I guess.
Overall, I'd say it is more of an issue for the person using the mods than the person making the mods. If the balance is thrown off to be too common or rare, it would probably be easier for the user to use an additional mod like Custom Chest Loot to balance it as wanted.

I'm wondering how weights work exactly-- it it % chance to get picked for an item slot? Like each chest can have a random # of items in it, and the weights are run on each item slot. I guess if there is a way to get total weights I could just use % chance? If it's per item slot, I think around 5% chance for heart container and 10% chance for 1-3 heart pieces sounds reasonable.

So first, use the /nbtedit me command, then open up the attributes section.
Open up the (TagCompounds) until you see generic.maxHealth. what is the "base" value for that?
--The base value should be 20.
Then open up the modifier section in the TagCompound with generic.maxHealth.
More (TagCompounds) should popup, open them until one with name "More Health Heart Modifier". What is the value for this?
--This value should be -10 (health) for a new player, with 5 starting hearts.
Basically, minecraft starts with maxhealth=20=10 hearts. SInce you set starting hearts to 5 (10 health), my modifier would be -10 so the maxhealth shown would be 10= 5 hearts, without having to change the maxhealth attribute itself.
I think the issue is before this update I manually changed the maxhealth attribute instead of using modifiers, so updating to beta means those old attributes stuck.
So if your old player had say 7 hearts, that would mean he had 14 health. This would have been the maxhealth stored, so -10 would bring him down to 4 health=2 hearts.
I can fix this issue by not using the maxHealth variable, but instead just 20. Hopefully other mods won't modifier the maxHealth variable directly, and will use modifiers like I should have.
--I will also add a variable config option to force everyone to update to their correct values (based on how much current XP they have-- heart container values are lost). That can be turned on if issues still persist.

Well you're pretty close to how I know it to work. Just to note, chances are I may be a tad off in some part of my explanation. It seems every few weeks I find something new about how chests work, due to updates or other things XD
The weights determine the item's percent chance based on the whole or total. So if the total weight for all the items is 10, an item with 5 weight has a 50% chance to be picked. If the total weight of all items is 100, it has a 5%. If the total weight is 100 and the item is 10, then it is 10% chance. I think that is enough examples to get the point.
So it will be tricky to set an appropriate weight for an item since you can't be sure what the total weight is. You can base it off the vanilla chest weights, which is probably the best choice. But other mods can still throw that off, because any addition would increase the total weight and lower the chances of all the items. Still, it is probably best to make the items rarer than too common, since the multiplier should allow players to make them more common if they do get greatly lowered.
As for how many items are picked, each chest has a set count of items it will pick from. For the dungeon chest, it seems to be 8-8. So every time it picks 8 items based on their chances, but sometimes you get less than 8 items because it picks the same chest slot twice and overwrites the previous item (or at least I believe that is why).

Also people above 20 health (10 hearts) take damage randomly to ensure they stay at 20 health. They will constantly regenerate their health only to take random damage, annoying because it makes the players hunger go down really fast and basically makes the extra hearts worthless.
Hmm, wonder why it's displaying 0 for you. It seems like the modifier is working for you, since the base is definitely 20, and you actually have 10 health. Perhaps it just won't show negatives.
Is it possible to get the nbt info from the players experiencing this bug?
Since my mod has been around for several years now, I have to continuously adapt. Some old code probably is messing things up. I'll have to brush it up.
Unless I can get the information from my side then no I can't get ht nbt information sadly.
EDIT:
Another issue that is happening to 1 new player is that his hearts slowly go away until he is at zero and then is in a constant stream of death. I fixed this slightly by deleting his player data and then tossing exp bottles and health potions at him until he earned 2-3 hearts. He then proceeded to lose 1 or 2 of those earned hearts but by the end of it he had enough health to survive.
