What do width and height represent here? (it can't just be the image size)
I'm pretty sure that the "width" and "height" are tiles. Meaning, you don't have to have all the frames set up in a 1 by n vertical strip. If your width is 2 and your height is five, then you have 10 total tiles arranged 2 x 5 on your image.

I'm quickly learning to take anything the wiki says about resource packs with a grain of salt. This is like the third time where the wiki has been blatantly wrong about a .mcmeta feature.
I'll give it a try a little later on just to make sure, but it wouldn't surprise me if this was some editor trolling again.
