1: The thought of being completely, utterly forgotten. I know that will almost certainly never happen as long as I live, but the thought of it is still terrifying.
2: The concept of eternity. The thought of something continuing on FOREVER without an end is rather ominous. Especially when applied to something negative.
3: Maggots. Not the scariest thing in the world, but it's still unnerving to watch them squirm the way they do.

Living forever- be it in a soul-like religious sense, or a more hollywood immortal sense- I don't like either.
At the same time I'm afraid of death, I guess I just want more time period.
Being defeated by something I cant even see, this applies to disease, microbe/ parasite/ viral related death, the enemy hiding behind the wall.
I'm afraid of slow death- cancer, dementia (alzheimers), death of old age, I'd rather die an instantaneous death than a slow and drawn out one.
Computers- yes, I love to play with them in every sense, but their capabilities and uses also scare me a lot.
The singularity.
Beings that are more evolved/ capable than I, selfish yes, but also disturbing (i.e. aliens). Also, beings of a higher dimensional plane (because it's obvious they exist).
I'm sure there's more to put on this list, somewhere in my mind...

I can understand most of your fears, but you cannot worry about death. It comes to all of us, eventually. I learned not to fear it years ago. Life is too short to worry about anything.
