I just had one a couple seconds ago. I was trying to build an xp farm that drops mobs to get them low, but for some reason they were always dying, even though when I tested it I was living with 2 hearts. After more than half an hour I realized it was because I was wearing Protection armor. Derp.

Long story short...
*begins to build big subterrannian city for my villagers*
*builds breeder underground*
"Why isnt it working!?"
that goes on for like 2 hours, then I read basically
"Villagers count homes based on number of exposed sun "outside blocks" and blocked sun blocks. If they are the same, it is not a house."
*Facepalm*
EDIT:
Oh, and my first level 30 enchanted diamond sword on my 1.4 world got Bane of Arthropods IV... (I can already take out spiders well)

- Exploring my world
- Been exploring for about 5 hours (Over a few days)
- Inventory full of Wool, Diamonds, Emeralds, various Dyes that I need and highly-enchanted tools
- Returning home
- travelling across HUGE ocean biome
- Traveled for about 5,000 ocean blocks, no sign of land
- Boat crashes into Squid, breaks on impact
I had no wood, no idea where the nearest land was. I swam about 1,000 blocks and decided I was way too bored. Swam underwater, dug into a cave and travelled underground for the rest of it. Took ages.

Hiding under a block at the bottom of a river - in a version where you take damage from being pushed into the wall. Also, getting rid of the water next to your sugarcane because the sugarcane keeps falling in.

Stop right beside a lava lake, get shot in the back by a skeleton, land on a 1x1 gravel pillar, proceed to walk off pillar and into lava, die.

*When I'm mining on the wrong level for diamonds and then I wonder why I can't find any
*Any time I leave my house, get really far and then realize I forgot something (i.e. food when going caving)
*When I accidentally get turned around exploring then look up at the sun to realize I'm going the wrong direction.
*When I accidentally craft the wrong item OR hit shift to get a bunch of an item when I only wanted one or two.

Spend four or five hours building something really epic, and then have a friend walk up to me and go, "You realize that you built that whole thing backwards right?" ... Facepaw.
Or the ever popular, *walk into new house, walk upstairs, get blown up by the creeper that spawned there because you FORGOT TO PUT LIGHTS IN.*

I was in the Nether (with tons of important crap that I always bring with me ) I was walking in what can be considered a "cave" and I fell into a VERY obvious hole in the ground and fell to my very sorry death.

A ghast appeared and shot me twice before turning away. He missed both times, and I called him a "blind idiot box." I hate ghasts so much. Then, I heard the both of the fireballs explode, and I turned around to see what happened. My glass window was busted open, and my portal was out... What are the chances of that!? It took me an hour or so to get some fire charges from ghasts, blazes, and wither skeletons. (Blaze powder + Gunpowder + Coal = Fire charge.)

I was mining and i found diamonds over a lava pool so, i though turn the lava into obsidian using water.I place water using my bucket and it pushes me into lava and i lose my diamonds
