I place this mod into my mods folder , but when im playing minecraft, i dont seem to have this item in my game!); I checked from toomanyitems and still nothing! ); Any help pls? and yea i alr have forge); AWESOME MOD YEA(:

Do you have this mod AND the base?
Alright i was able to get the mod but my skin character and some other items like the ender chest are looking very weird

You should add an animation for when the sword is inserted or pulled. I just think that an Ocarina of Time like animation would be so awesome. Something like it goes 3rd person and shows you pulling it out of the ground and pointing it skyward (and for the returning of the blade, going 3rd person, raising the sword up and smashing it into the pedestal) would just be astonishing and beautiful. The OoT music that plays when pulling the sword being implemented would be badass as well.

While I agree this would look great, it would just annoy people. Especially in Multiplayer.

It would also require making use of player render API, and could make it conflict with smart moving.  Personally, I would prefer to keep my crawling along the ground Link-style, vs lose that for a pretty animation... that would get annoying.

Maybe you could make that animation optional, as in you can make it so that players can turn the animation on or off. And you could make it off on Multiplayer by default.

Just checked and the mod works with 1.4.5 in it's current state. Just get new Forge.

That's how you make a spoiler.
Now then, if what I'm reading is right (and it looks pretty straightforward so I don't know why the conclusion wouldn't be right), the problem resides in the Randomobs part of MCPatcher. Taking a wild guess, either you installed something wrong, in the wrong order, or one of these mods doesn't like the other.
