Due to not having much time to maintain this mod and my other mods I am discontinuing this mod, I have officially passed the maintenance of this mod onto fergoman007, but you can also find an unofficial update here.
UseCount was originally made by Coolblinger, he has announced that he will not be continuing the mod, and so it has been passed to me to continue and maintain.
Information
UseCount is a simple mod that adds a counter above the durability bar of an item showing the remain uses of the item.
Many requests where made for the original mod to be continued after Coolblinger stopped so I have updated and re-wrote the mod and will continue to maintain the mod as my own for now.
1. Navigate to your .minecraft folder located at %APPDATA%\roaming\.minecraft.
2. Locate your minecraft.jar and open it using an archiving program like WinRar or 7Zip.
3. Download Minecraft Forge v6.5.0.471 or later and UseCount 2 v4.4 or later.
4. Open the Minecraft Forge .zip and place the contain files inside the minecraft.jar, overriding all files.
5. Open UseCount .zip and place all contained files inside the minecraft.jar, overriding bce.class.
6. Delete the META-INF folder.
Mac

5. Add both Minecraft Forge and UseCount .zips to the minecraft.jar tab, ensure UseCount is place after Minecraft Forge in the load order.
6. Click Ok and Launch the instance to install the mods and play.

Forge is Required as I mention in the OP, if you want or require a non forge version the original mod (Coolblingers version) is still available for download as far as I know.
However this will not have Vazkii's Update Manager implemented and thus you will not be notified of updates, it will also not be compatible with 3D Items or be updated to any new versions of Minecraft.
Though if enough people request it I will host a non-forge version.

As N00b_Warrior said the crash log shows that TMI is causing the crash, UseCount only edits the awg.class and having a look at the TMI files this is not one that it edits so it isn't that the file is being overridden.
Also as can be seen UseCount isn't even loaded by the time the crash happens, only modloader is loaded and then the crash occurs in TMI, i would suggest trying again from the start and checking that you have put the files in the correct places and in the correct orders.
EDIT: just tested and Modloader, TMI and UseCount work fine together for me
